Records <strong>of</strong> a law enforcement unit <strong>of</strong> the university which are maintainedapart from educational records solely for law enforcement purposes and arenot disclosed to individuals other than law enforcement <strong>of</strong>ficials <strong>of</strong> the samejurisdiction;c. Records relating to an individual who is employed by the university whichare made and maintained in the normal course <strong>of</strong> business, relate exclusivelyto the individual in that individual‟s capacity as an employee, and are not foruse for any purpose (this sub-paragraph does not apply to records relating toany individual in attendance at the university who is employed as a result <strong>of</strong>his or her status as a student);d. Personal health records <strong>of</strong> a student, which are created or maintained by apr<strong>of</strong>essional acting in his or her pr<strong>of</strong>essional capacity, used only inconnection with the provision <strong>of</strong> treatment to a student, and not disclosed toanyone other than individuals providing the treatment (provided that therecords can be personally reviewed by a physician or other appropriatepr<strong>of</strong>essional <strong>of</strong> the student‟s choice); ande. Alumni records compiled after graduation.5. "Personally identifiable" means that the data or information includes:a. The name <strong>of</strong> a student, the student‟s parent or other family member;b. The address <strong>of</strong> the studentc. A personal identifier, such as the student‟s social security number or studentnumber;d. A list <strong>of</strong> personal characteristics which would make the student‟s identityeasily traceable; ore. Other information which would make the student‟s identity easily traceable.6.
