Students are assigned faculty advisors at orientation.Advisors meet with first and second year students twice minimum, usually afterblock II exams, once in the fall and once in the spring.The student will fill out an advising form located on the Advising andPr<strong>of</strong>essionalism page on the TU website and bring it to their meeting, along witha printout <strong>of</strong> their latest grade report. This will be an opportunity for discussion<strong>of</strong> any concerns by either party. At the end <strong>of</strong> the second year, the advisor willprovide a summary statement <strong>of</strong> his/her observations concerning the student‟sprogress and development through the first two years <strong>of</strong> the program.Completed advising forms are submitted to the <strong>of</strong>fice <strong>of</strong> the Associate Dean forPreclinical Education, who will monitor the advising program, noting if advisorsare meeting with students and completing the advising forms.Students may change their advisor once. They will need to fill out a change <strong>of</strong>advisor form located on the Advising and Pr<strong>of</strong>essionalism page on the TUWebsite and submit it to the <strong>of</strong>fice <strong>of</strong> the Associate Dean <strong>of</strong> Preclinical Education.An email from the student must also be sent indicating their intent to switchadvisors. This email must include the old and new advisor, as well as theAssociate Dean <strong>of</strong> Preclinical Education.One faculty member will be assigned the role <strong>of</strong> Vice-Chair for Pr<strong>of</strong>essionalismand will serve as an <strong>of</strong>ficer <strong>of</strong> the Student Promotion Committee.The responsibilities <strong>of</strong> the Vice-Chair for Pr<strong>of</strong>essionalism include: reviewingPr<strong>of</strong>essionalism Report Forms (see below); acting as a liaison with the AssociateDean <strong>of</strong> Student Services to ensure students‟ rights <strong>of</strong> due process are protected;bringing before the Student Promotion Committee any students about whomthere are pr<strong>of</strong>essionalism concerns and chairing that part <strong>of</strong> the meeting, andserving as a resource to report pertinent positive or negative pr<strong>of</strong>essionalismcomments on file for all other students who come before the committee.Faculty should notify the Vice-Chair for Pr<strong>of</strong>essionalism if there are any concernsregarding pr<strong>of</strong>essionalism in any student.Pr<strong>of</strong>essionalism Report Forms will be made available for anyone on campus tosubmit, to report positive or negative examples <strong>of</strong> pr<strong>of</strong>essionalism.<strong>2011</strong> – <strong>2012</strong> COM Student Handbook
