ACCOMMODATIONSAn application for accommodation <strong>of</strong> a disability must be made by the student. Verbaldisclosure prior to or following admission is not sufficient. Students may apply forspecial accommodations anytime during their academic curriculum, however, ifgranted, the accommodation is given only for the academic year in which it isrequested. In case <strong>of</strong> changed circumstances, with respect to any disability, subsequentapplications must follow for each academic year the student is requestingaccommodations (including request during clinical rotations, clerkships, internships orany other <strong>of</strong>f-campus programs that are part <strong>of</strong> the students‟ required college/programcurriculum).A student requesting an accommodation for a disability under the ADA must meet withthe Associate Dean <strong>of</strong> Student Services. An application for accommodation <strong>of</strong> adisability can be filled out in this <strong>of</strong>fice.Although students can apply for accommodations at any time during their academicprogram, it is strongly advised that if you are seeking accommodations, you have thefollowing paperwork and documentation on file in the Office <strong>of</strong> Student Services priorto your request.1. Students must fill out the Request for Accommodations <strong>of</strong> Disability Application(RADA) prior to consideration <strong>of</strong> such requests. To expedite the process,students are encouraged to have this form prior to their meeting with theAssociate Dean <strong>of</strong> Student Services. The form is available in the Office <strong>of</strong> StudentServices.2. Documentation is required from a physician, clinical psychologist or othercertified individual and a report must be on file in the Office <strong>of</strong> Student Servicesbefore any accommodation can be granted. It is the student‟s responsibility tomake sure that all required documentation is forwarded to the Office <strong>of</strong> StudentServices. Documentation should include:a. A cover letter from the physician, clinical psychologist or other certifiedindividual.b. Appropriate and thorough diagnostic test results. Summarization <strong>of</strong>results, while helpful, by itself is not considered satisfactory. The reportshould also a) clearly diagnose the disability, b) report the severity <strong>of</strong> thedisability, c) <strong>of</strong>fer implications <strong>of</strong> the disability for the students‟ program<strong>of</strong> study, and d) include a summary <strong>of</strong> what accommodations are neededto assist in overcoming the disability.<strong>2011</strong> – <strong>2012</strong> COM Student Handbook
