Students, while having a specificdisability and requesting accommodations for one course, may or may not want specialaccommodations for other courses within their program <strong>of</strong> study. The wordingprovided in the written letter from the Associate Dean <strong>of</strong> Student Services toDepartment Chairs or Program Directors will state, "If requested, the followingaccommodations are recommended…"Students are encouraged to immediately bring problems to the attention <strong>of</strong> AssociateDean <strong>of</strong> Student Services. It is the responsibility <strong>of</strong> the Associate Dean <strong>of</strong> StudentServices to monitor all arrangements regarding disability accommodations. If a studentfeels he or she is being discriminated against because <strong>of</strong> a disability, he or she has theright to request an investigation into such a matter through the stated grievance policiesand procedure in Appendix B <strong>of</strong> this Handbook.Students who are in need <strong>of</strong> testing or who need to provide current documentationshould meet with the Associate Dean <strong>of</strong> Student Services. Information on testing andtesting locations can be obtained through this <strong>of</strong>fice. Costs <strong>of</strong> testing and assessmentsare the responsibility <strong>of</strong> the student.If students have gone through the proper procedures and the request is denied, theymay request an appeal from the Committee on Disabilities which is an ad-hoccommittee that will be established by the Office <strong>of</strong> Student Services. The request forappeal must be made in writing within 30 days <strong>of</strong> notice <strong>of</strong> receiving the initial decision“to deny." The decision <strong>of</strong> the Committee shall be rendered in ten (10) business daysand will be considered final.Access to these records is restricted. If the accommodating departments (s),instructors(s), wish to view a copy <strong>of</strong> the student assessment, they may do so only withthe express written consent <strong>of</strong> the student and must show reason why they wish toview the assessment. Copies <strong>of</strong> the detailed assessment are maintained in a confidentialfile in the Office <strong>of</strong> Student Services and may only be viewed by this <strong>of</strong>fice andindividuals that the student has identified.Students who wish to request accommodations for board examinations or othercertifying examinations are urged to contact the governing body or institutionresponsible for the administration <strong>of</strong> such examinations.
