II.III.Requests for reproduction <strong>of</strong> a transcript must be made in writing tothe <strong>University</strong> Registrar on the appropriate form to be obtained fromthe Office <strong>of</strong> the Registrar.Requests for reproduction <strong>of</strong> other records must be made in writing tothe appropriate <strong>of</strong>ficial holding the desired records.IV. All reproduction requests must be accompanied by the payment <strong>of</strong>record reproduction charges.V. Reproduction shall be done as soon as feasible, but not to exceed 45days after receipt <strong>of</strong> the request.Amendment <strong>of</strong> Education Records1. Request <strong>of</strong> education recordsa. A student who believes that information contained in his/her educationrecords is inaccurate or misleading, or violates privacy or other rights, mayrequest that the records be amended.b. Such a request shall be made in writing and submitted to the custodian <strong>of</strong> thedisputed record, who shall decide whether to amend the record in accordancewith the request within a reasonable period <strong>of</strong> time (not exceeding 45 days) <strong>of</strong>receipt <strong>of</strong> the request.c. If the custodian <strong>of</strong> the disputed record, after consultation with the Dean <strong>of</strong> theindividual college, refuses to amend the record in accordance with thestudent‟s request, he/she shall immediately notify the student in writing <strong>of</strong>such refusal and advise the student <strong>of</strong> the right to a hearing under subsectionIII.2 below.2. Right to and conduct <strong>of</strong> hearinga. If the request by a student to amend education records is denied, he/she may,in writing submitted to the custodian <strong>of</strong> the record within 45 days after thedenial, request a hearing in order to challenge the content <strong>of</strong> the records toinsure that information therein is not inaccurate, misleading or otherwise inviolation <strong>of</strong> the privacy or other rights <strong>of</strong> the student.b. The hearing shall be conducted before a review board composed <strong>of</strong> onemember <strong>of</strong> the faculty who has no direct interest in the outcome <strong>of</strong> thehearing, one administrator <strong>of</strong> the university (appointed by the CEO <strong>of</strong> the<strong>2011</strong> – <strong>2012</strong> COM Student Handbook
