1. Application for graduation by the student and audit <strong>of</strong> progress towardgraduation requirements by the Office <strong>of</strong> the Registrar;2. Audit <strong>of</strong> records in the Office <strong>of</strong> the Bursar, indicating that all tuition, fees andfines (if any) are paid;3. Confirmation by the Office <strong>of</strong> Financial Aid that the Exit Interview is complete,and4. Review <strong>of</strong> records and final approval to release the diploma indicated by thesignature <strong>of</strong> the Dean or Dean‟s designee (Assistant Dean, Associate Dean, orProgram Director).Student Academic Integrity and EthicsRESPONSIBILITY OF THE ADMINISTRATION & FACULTYThe administration should provide physical settings and examination format thatminimizes academic misconduct. Crowded examination conditions should be avoidedand alternate seating should be provided during examinations when possible. Inaddition, the administration should appoint adjudication committees consisting <strong>of</strong>faculty and students to deal promptly and with procedural fairness with cases <strong>of</strong>alleged academic dishonesty. The faculty should clearly inform students <strong>of</strong> gradingrequirements and performance expectations for each assignment as well as useexamination formats that discourage academic misconduct.RESPONSIBILTY OF THE STUDENTTo be honest, act fairly towards others, take group and individual responsibility forhonorable behavior and know what constitutes academic dishonesty.CODE OF CONDUCT / PROFESSIONAL PROMISEStudents are expected to emulate the legal, moral and ethical standards expected <strong>of</strong>pr<strong>of</strong>essionals in their respective areas and display behavior which is consistent withthese qualities. Pr<strong>of</strong>essionalism and pr<strong>of</strong>essional ethics are terms that signify certainscholastic, interpersonal and behavioral expectations. Among the characteristicsincluded in this context are the knowledge, competence, demeanor, attitude,appearance, mannerisms, integrity, morals, etc. displayed by the students to teachers,peers, patients and colleagues in the other health care and educational settings.<strong>2011</strong> – <strong>2012</strong> COM Student Handbook
