Students enrolled in programs at TUC and involved in patient-related activities arerequired to wear a name tag which clearly shows their name and identifies the programand college in which they are enrolled. An acceptable format is as follows:John SmithTUCOM<strong>Osteopathic</strong> Medical StudentStudent RecordsDISCLOSURE AND ACCESSS TO RECORDSStudents who are currently or have been previously enrolled at the institution have theright to inspect and review certain educational records and to withhold from releasecertain personally identifiable information in accordance with the Federal FamilyEducational Rights and Privacy Act (FERPA) <strong>of</strong> 1974. This policy regarding studentaccess to educational records does not include the right to review such items asconfidential letters and statements <strong>of</strong> recommendation if the student has waived theright to inspect and review those recommendations.DISCLOSURE OF STUDENT DIRECTORY INFORMATIONThe <strong>University</strong> designates the following personally identifiable items as StudentDirectory Information: Student name, address, telephone number, e-mail address, dateand place <strong>of</strong> birth, major field <strong>of</strong> study, dates <strong>of</strong> attendance, degrees and awardsreceived, most recent previous school attended and photograph. The <strong>University</strong> maydisclose any <strong>of</strong> those items listed above as public information without prior writtenconsent, unless notified in writing to the contrary by December 31 <strong>of</strong> each academicyear. Other confidential personal information not listed above as Student DirectoryInformation may not be disclosed to third parties without the prior written consent <strong>of</strong>the student, except under certain circumstances. These exceptions include, but are notlimited to, disclosure to institutional <strong>of</strong>ficials performing assigned duties related to theeducational or accreditation interests <strong>of</strong> the <strong>University</strong>, agencies verifying the financialaid status <strong>of</strong> the student, parties involved in health or safety emergencies related to thestudent or others, and certain law enforcement, legal or judicial authorities.PROCEDURE FOR DISCLOSURE AND ACCESS TO STUDENT RECORDSDisclosure and access to a student's educational records may be requested throughinstitutional policy as follows: A student desiring to inspect and review his or herrecords must submit a written request directly to the person in charge <strong>of</strong> the desired<strong>2011</strong> – <strong>2012</strong> COM Student Handbook
