Page 1 <strong>of</strong> 6ACTUAL PROGRAM OUTPUTS REPORT INSTRUCTIONSRead these instructions carefully prior to completing the Actual Program Output Report.The purpose <strong>of</strong> the Actual Program Output Report is to gather information on how CSCDs haveused <strong>TDCJ</strong>-<strong>CJAD</strong> funds for services, interventions, and programs. Do not complete the ActualProgram Output Report to indicate how Basic Supervision funds are used for basic supervision(i.e., paying general probation staff, urinalysis testing on all probationers). An Actual ProgramOutput Report must be completed if:1. A program proposal was submitted as part <strong>of</strong> the Community <strong>Justice</strong> Plan (CJP) and <strong>TDCJ</strong>­<strong>CJAD</strong> funded the program with Diversion Program (DP) or Community Corrections (CC)funds.If a program proposal is submitted with secondary program codes, a separate ActualProgram Output Report will be required for each primary program code AND eachsecondary program code. In general, the number <strong>of</strong> participants reported for eachsecondary program code should be less than or equal to the number <strong>of</strong> participants reportedfor the primary program code.In addition, in cases <strong>of</strong> combination facilities, separate Actual Program Output Reportsshould be completed for each facility type, even if only one program proposal is in theCommunity <strong>Justice</strong> Plan.2. A program proposal was submitted as part <strong>of</strong> the CJP for a Community Corrections Facility(CCF) funded through Treatment Alternatives to Incarceration Program (TAIP) orResidential Substance Abuse Treatment (RSAT) monies. (Do not submit forms for TAIPfundedContract Residential Service [CRS] programs.)-or­3. <strong>TDCJ</strong>-<strong>CJAD</strong> Basic Supervision funding was used to support residential or non-residentialservices. In this case, the CSCD may not have submitted a program proposal in theCommunity <strong>Justice</strong> Plan, but Basic Supervision funds were used to operate the program,service or facility.The three areas above should cover all programs funded with Community Corrections (CC),Diversion Program (DP) and Basic Supervision funds.- 54 ­
