1973 cont Social Security Act (c.38) set out reforms <strong>and</strong> introduced earnings related<br />

contributions; annual reviews for contributions <strong>and</strong> benefits; <strong>and</strong> phased out the graduated<br />

pensions scheme. See 1974 <strong>and</strong> 1975.<br />

The Royal Commission on the Constitution, Cmnd.5460, (chairman, Lord Kilbr<strong>and</strong>on),<br />

set out a number <strong>of</strong> possible schemes for devolving power to regions <strong>of</strong> Britain. See 1975.<br />

The Committee on "The New Scottish Local Authorities: Organisation <strong>and</strong><br />

Management Structure" (chairman, IV Paterson) recommended corporate management <strong>and</strong><br />

the appointment <strong>of</strong> a chief executive.<br />

A White Paper "Private Practice in National <strong>Health</strong> Service Hospitals", Cmnd.5270,<br />

considered "private practice operates to the overall benefit <strong>of</strong> the NHS" <strong>and</strong> that no major<br />

changes to the system were needed, but control procedures in hospitals should be strictly<br />

observed. See 1976.<br />

The Working Party on Collaboration between the NHS <strong>and</strong> Local Government<br />

(chairman, ARW Bavin) called for a statutory obligation for the authorities to collaborate to<br />

secure the health <strong>and</strong> welfare <strong>of</strong> the people <strong>of</strong> their areas <strong>and</strong> to set up joint consultative<br />

committees; <strong>and</strong> for local authorities to co-opt members <strong>of</strong> the health authorities to their<br />

relevant committees. It suggested that both authorities should have full powers to provide<br />

goods <strong>and</strong> services to each other, <strong>and</strong> that health authorities should second, on the invitation<br />

<strong>of</strong> a district council, a medical <strong>of</strong>ficer to act as adviser <strong>and</strong> "proper <strong>of</strong>ficer" on environmental<br />

health matters to that council. The report set out detailed proposals for the future organisation<br />

<strong>of</strong> the school health service by the health authorities. See 1974.<br />

The Sub-Group on the Organisation <strong>of</strong> a Medical Advisory Structure (chairman, NGC<br />

Hendry) <strong>of</strong> the Scottish Joint Working Party on the Integration <strong>of</strong> Medical Work considered<br />

that such a structure should provide advice to management; that there should be clearly<br />

defined lines <strong>of</strong> communication <strong>and</strong> access to full information so that every doctor may<br />

participate in the advisory process with full knowledge <strong>and</strong> underst<strong>and</strong>ing; <strong>and</strong> that there<br />

should be close liaison with the nursing pr<strong>of</strong>ession <strong>and</strong> others concerned with the care <strong>of</strong><br />

patients. The Sub-Group recommended that community medicine specialists should be<br />

involved in every activity <strong>of</strong> the medical advisory structure.<br />

Another Sub-Group <strong>of</strong> the Scottish Joint Working Party on "Towards an Integrated<br />

Child <strong>Health</strong> Service" (chairman, Sir John Brotherston, see 1967) recommended that all<br />

doctors with a substantial commitment to child health should form a paediatric division, <strong>and</strong><br />

that all such doctors should have special training. See 1976.<br />

A third Sub-Group <strong>of</strong> the Joint Working Party in its report "Community <strong>Medicine</strong> in<br />

Scotl<strong>and</strong>" (chairman, JL Gilloran, 1916-95, medical <strong>of</strong>ficer <strong>of</strong> health, Edinburgh) made<br />

recommendations about the nature <strong>and</strong> practice <strong>of</strong> community medicine; the organisation <strong>of</strong><br />

the specialty; <strong>and</strong> the future pattern <strong>of</strong> its training. See 1974.<br />

The Sub-Group on "General Practitioners in the Hospital Service" (chairman, J Crooks)<br />

recommended pilot schemes <strong>of</strong> general practitioner units in district general hospitals <strong>and</strong> <strong>of</strong><br />

"combined practice" by general practitioners <strong>and</strong> consultants.<br />

The Scottish Home <strong>and</strong> <strong>Health</strong> Department published reports on "The Maternity<br />

Services: the Integration <strong>of</strong> Maternity Work", <strong>and</strong> on "The Future <strong>of</strong> the Chest Services in<br />

Scotl<strong>and</strong>" (chairman, JW Cr<strong>of</strong>ton). See 1997.<br />

The Committee on Hospital Complaints Procedure (chairman, Sir Michael Davies)<br />

recommended a detailed code <strong>of</strong> practice; the establishment <strong>of</strong> investigating panels to deal<br />

with serious complaints; <strong>and</strong> the involvement <strong>of</strong> community health councils in advising <strong>and</strong>
