Short-Term Stabilization in Inpatient Psychiatric Settings:<br />

A Necessary Component of Systems of Care<br />

OBJECTIVES—Participants will learn:<br />

1. To identify key concepts in developing programming <strong>for</strong> standard short inpatient stays with a focus on specific<br />

objectives <strong>for</strong> assessment and treatment planning<br />

2. How to reduce recidivism in inpatient care by using transition planning <strong>for</strong> post-discharge care and a system of care<br />

approach to practice<br />

3. How to adapt principles that guide a system of care approach to inpatient programming<br />

4. How a family-driven, youth-guided approach using collaborative problem solving can be successfully incorporated<br />

into a fast-paced inpatient medical setting<br />

This Workshop will focus on a model <strong>for</strong> a short-term inpatient stay with specific and achievable objectives combined<br />

with a strong focus on transition planning that provides an effective approach when inpatient care is necessary. The<br />

approach is based on system of care principles such as family-driven, youth-guided care, with the goal of reducing<br />

recidivism and improving intermediate-term and long-term functioning.<br />

Faculty will highlight the organizational change required to implement this approach; the program components; and<br />

required support services. The model shifts the focus of short inpatient stays to accurate assessment of the child and<br />

caregiving system rather than on treatment per se, and is based on the premise that ongoing treatment and outpatient<br />

stabilization require strong post-discharge planning from the first day of hospitalization that relies on family and child<br />

input and utilizes in<strong>for</strong>mal and <strong>for</strong>mal supports. The approach outlines specific objectives <strong>for</strong> all team members and the<br />

child and family over an average seven-day inpatient stay.<br />

The strategies to be presented are based on an innovative program in Richmond, Virginia that illustrates application<br />

of system of care-in<strong>for</strong>med practice into inpatient care within the continuum of care <strong>for</strong> children with mental<br />

health conditions.<br />

Specific topics to be covered include:<br />

• Specific daily objectives tailored to a short-term stay inpatient program emphasizing an accurate assessment of the<br />

clinical problem<br />

• Strategies <strong>for</strong> developing a post-discharge treatment plan that is based on thorough understanding of the problems<br />

and in conjunction with engaged caregivers and youth<br />

• How to create a treatment plan with transition or bridge to outpatient community settings<br />

• How to create a youth-guided, family-driven wraparound post-discharge plan with shared ownership<br />

that improves engagement<br />

• How to measure outcomes such as reduction of recidivism, improvement of school attendance/per<strong>for</strong>mance, and<br />

family satisfaction<br />

The model will be illustrated through role play and audience participation. Faculty will include the perspective of a<br />

medically trained person who has changed her practice by embracing a system of care approach.<br />
