WORKSHOPS<br />
The basis <strong>for</strong> the strategies to be taught come from the real life experiences of the youth themselves and the guidance<br />
they received from key individuals in their lives. Specific topics to be covered include:<br />
• Helpful suggestions <strong>for</strong> working with youth who are in the process of discovering their cultural and sexual identities<br />
• Critical elements that can change the course of successful navigation of identity development<br />
• The complexity of cultural and sexual identity development that is compounded by mental health challenges due to<br />
life circumstances, such as involvement with the child welfare system<br />
• Specific advice will be offered to parents/caregivers, professionals, and other youth about how to best provide support<br />
and guidance<br />
The youth panel will present their own individual PowerPoint presentations that are a combination of pictures, narrative,<br />
music lyrics, poems, quotes, and self-expression to tell their stories and communicate the major themes they wish the<br />
audience to take home. The audience will have an opportunity to interact with the panel members during the<br />
presentation. Additionally, we will have the capacity <strong>for</strong> live tweeting by the audience during the presentation to hear<br />
your reactions/responses live to what the youth panel is sharing and provide a basis <strong>for</strong> our interactive discussion. The<br />
faculty team will primarily share the perspectives of the youth and young adult panel members.<br />

Integrating a System of Care Approach into Pediatric Medical Homes<br />
OBJECTIVES—Participants will learn:<br />
1. To define the term pediatric medical homes and to describe the underlying principles that align with the system of<br />
care approach<br />
2. To identify key structural components of a team-based model that integrates mental health and primary care<br />
3. To identify concrete strategies and tools <strong>for</strong> engaging primary care providers in a system of care through application<br />
of an evidence-based quality improvement framework<br />
4. To identify policy issues pertaining to the pediatric medical home as a hub <strong>for</strong> integrated behavioral health services<br />
This Workshop will focus on practical strategies <strong>for</strong> integrating systems of care and pediatric medical homes,<br />
identifying components of an integrated model and factors related to sustainability of this approach. Faculty will<br />
highlight strategies <strong>for</strong> integrating behavioral health and primary care including embedded clinical services, provider<br />
trainings, integrated governance councils, and evidence-based quality improvement processes. Faculty will also discuss<br />
the sustainability of pediatric medical homes as a hub <strong>for</strong> sustainable, integrated behavioral health services in an<br />
environment of changing healthcare systems.<br />
The approaches to be presented are based on the work of the Boston Public Health Commission and Massachusetts<br />
Executive Office of Health and Human Services in piloting MYCHILD—a model that embeds an early childhood<br />
mental health clinician and family partner into three pediatric medical homes and the Boston Healthcare <strong>for</strong> the<br />
Homeless program.<br />
Specific topics to be covered include:<br />
• The evolution and principles of pediatric medical homes<br />
• A rationale <strong>for</strong> linking systems of care with pediatric medical homes, emphasizing the natural alignment of core<br />
principles related to family-driven, culturally competent, coordinated care between the two frameworks<br />
• Structural components of a model that integrates mental health into pediatric medical homes, including<br />
multidisciplinary teams, embedded family partners and clinicians, family-driven care plans, and a quality<br />
improvement process<br />
• Application of an evidence-based quality improvement process, a Learning Collaborative, to facilitate the integration<br />
of mental health and primary care<br />
