Dear Mr Smith<br />

On 12th June last year, you were informed that you would be given a written<br />

warning in accordance with the second stage <strong>of</strong> the Company’s Disciplinary<br />

Procedure. In a letter <strong>of</strong> 15th July you were warned that unless your conduct<br />

improved, you were likely to be dismissed.<br />

At the disciplinary hearing held on 1st February, it was decided that your<br />

performance was still unsatisfactory and that you would be dismissed.<br />

I am therefore writing to confirm the decision that you be dismissed and that your<br />

last day <strong>of</strong> service with the company will be 15th February.<br />

The reasons for your dismissal are:<br />

You have the right to appeal against this decision to the Production Director<br />

within seven days <strong>of</strong> receiving this notice <strong>of</strong> dismissal, in writing, giving your<br />

reasons.<br />
