2.2 Questionnaires The collection tools used are based on the questionnaires developed by the MEASURE DHS project adapted for Senegal in agreement with the technical services of MSAS. During the SCSPA in Senegal 2012–2013, four types of data collection instruments were used: • Health Facility Inventory: This questionnaire includes a cover sheet to identify the facility and a list of staff to record all health providers present on the day of the interview. The inventory of the health facility is designed to obtain information on the ability of the facility to provide each of the priority services. It collects information on the availability of specific items (including their location and condition), the components of support systems (logistics, maintenance, and management), and the infrastructure of the facility (including the work environment where the services are offered). Resources evaluated are those required to provide the service according to internationally accepted guidelines and protocols. Services evaluated are those that are generally recognized as essential to ensure adequate support of the health services. • Health Care Providers Questionnaire: The health care provider questionnaire is designed to obtain information on individual qualifications of providers, the type of service they provide, and the type of training they received. A sample of health care providers is selected from the list of personnel to be interviewed using the health staff questionnaire. • Observation Checklists: These lists evaluate the level of adherence of providers to the standards of service quality based on internationally and nationally accepted guidelines and protocols. Components of the observation include the procedures used in conducting certain specific steps such as physical examinations and content of the conversation between provider and client (history, symptoms, and counseling). For this first year, clients from the family planning and sick children services were chosen for observation. • Exit Interview Questionnaires: The exit questionnaires are designed to assess the level of understanding by the clients of the services offered and concerned the consultation and examination and the instructions and counseling given with regard to treatment or preventive measures. Understanding of key messages by clients increases the likelihood of successful treatment or compliance with preventive behaviors, which promotes optimum results for their health. Client perception about the work environment was also evaluated. Exit interviews are conducted with clients whose consultations had been observed. • Special Questionnaire for Health Huts included the following: - an inventory of the health hut designed to assess the specific services of the hut - a review of notebooks, records, and documents related to family planning (FP), 5
