2.2 Questionnaires<br />
The collection tools used are based on the questionnaires developed by the<br />
MEASURE DHS project adapted for Senegal in agreement with the technical services of<br />
MSAS.<br />
During the SCSPA in Senegal 2012–2013, four types of data collection instruments<br />
were used:<br />
• Health Facility Inventory: This questionnaire includes a cover sheet to identify<br />
the facility and a list of staff to record all health providers present on the day of<br />
the interview. The inventory of the health facility is designed to obtain<br />
information on the ability of the facility to provide each of the priority services. It<br />
collects information on the availability of specific items (including their location<br />
and condition), the components of support systems (logistics, maintenance, and<br />
management), and the infrastructure of the facility (including the work<br />
environment where the services are offered). Resources evaluated are those<br />
required to provide the service according to internationally accepted guidelines<br />
and protocols. Services evaluated are those that are generally recognized as<br />
essential to ensure adequate support of the health services.<br />
• Health Care Providers Questionnaire: The health care provider questionnaire is<br />
designed to obtain information on individual qualifications of providers, the type<br />
of service they provide, and the type of training they received. A sample of health<br />
care providers is selected from the list of personnel to be interviewed using the<br />
health staff questionnaire.<br />
• Observation Checklists: These lists evaluate the level of adherence of providers<br />
to the standards of service quality based on internationally and nationally accepted<br />
guidelines and protocols. Components of the observation include the procedures<br />
used in conducting certain specific steps such as physical examinations and<br />
content of the conversation between provider and client (history, symptoms, and<br />
counseling). For this first year, clients from the family planning and sick children<br />
services were chosen for observation.<br />
• Exit Interview Questionnaires: The exit questionnaires are designed to assess<br />
the level of understanding by the clients of the services offered and concerned the<br />
consultation and examination and the instructions and counseling given with<br />
regard to treatment or preventive measures. Understanding of key messages by<br />
clients increases the likelihood of successful treatment or compliance with<br />
preventive behaviors, which promotes optimum results for their health. Client<br />
perception about the work environment was also evaluated. Exit interviews are<br />
conducted with clients whose consultations had been observed.<br />
• Special Questionnaire for Health Huts included the following:<br />
- an inventory of the health hut designed to assess the specific services of the hut<br />
- a review of notebooks, records, and documents related to family planning (FP),<br />
