Responsibility for this wide variety of tasks is divided among m<strong>in</strong>istries<br />

accord<strong>in</strong>g to various governmental decrees and these responsibilities were<br />

recently redef<strong>in</strong>ed (2010/7, 8). The primary responsibility for stewardship and<br />

f<strong>in</strong>anc<strong>in</strong>g <strong>in</strong> the health system rema<strong>in</strong>s with the State Secretariat for <strong>Health</strong>care<br />

(2010/8), but other m<strong>in</strong>istries are also <strong>in</strong>volved <strong>in</strong> service delivery and health<br />

care f<strong>in</strong>anc<strong>in</strong>g.<br />

2.3.3 State Secretariat for <strong>Health</strong>care<br />

In 2010 the government created the M<strong>in</strong>istry of National Resources by merg<strong>in</strong>g<br />

the five m<strong>in</strong>istries previously responsible for social, family and youth affairs;<br />

health care; education; culture; and sport (2010/7). These former m<strong>in</strong>istries have<br />

s<strong>in</strong>ce been reclassified as State Secretariats, each of which is led by a M<strong>in</strong>ister<br />

of State. The aim of this change was to reduce the cost of public adm<strong>in</strong>istration<br />

and create effective platforms for <strong>in</strong>tersectoral cooperation.<br />

With<strong>in</strong> the M<strong>in</strong>istry of National Resources, the State Secretariat for<br />

<strong>Health</strong>care is responsible for prepar<strong>in</strong>g legislation related to the direction of<br />

health care provision at the national level and at <strong>in</strong>stitutions of higher education,<br />

and for regulat<strong>in</strong>g national public health care tasks at the national level. The<br />

State Secretariat for <strong>Health</strong>care shares responsibility with the M<strong>in</strong>istry for<br />

National Economy and the M<strong>in</strong>istry of Interior for health care f<strong>in</strong>anc<strong>in</strong>g.<br />

The ma<strong>in</strong> functions of health policy formulation, coord<strong>in</strong>ation and<br />

regulation are carried out by a number of <strong>in</strong>stitutions under the direct control<br />

of the State Secretariat. In addition to these adm<strong>in</strong>istrative functions, some<br />

of these <strong>in</strong>stitutions provide health services themselves, <strong>in</strong>clud<strong>in</strong>g public<br />

health, emergency ambulance services, blood supply, tertiary care services<br />

and rehabilitation; they also help prepare health policy <strong>in</strong>itiatives. S<strong>in</strong>ce January<br />

2001, the State Secretariat has also exercised direct control over the NHIFA.<br />

One of the most important agencies of the State Secretariat for <strong>Health</strong>care,<br />

the NPHMOS provides public health services, <strong>in</strong>clud<strong>in</strong>g the traditional<br />

public hygiene and <strong>in</strong>fectious disease control, disease prevention and health<br />

promotion. It is the central authority responsible for implement<strong>in</strong>g, monitor<strong>in</strong>g<br />

and enforc<strong>in</strong>g regulations, <strong>in</strong>clud<strong>in</strong>g the registration and licens<strong>in</strong>g of health<br />

care providers. The NPHMOS is also responsible for monitor<strong>in</strong>g the quality of<br />

health services and plays an important role <strong>in</strong> capacity plann<strong>in</strong>g (see sections<br />

2.8.2, and 5.1 – 5.3).
