<strong>Health</strong> systems <strong>in</strong> transition <strong>Hungary</strong><br />
only <strong>in</strong> 1997 that the National Assembly regulated the mechanism to ensure<br />
quality <strong>in</strong> health care by sett<strong>in</strong>g up <strong>in</strong>ternal and external mechanisms (1997/20),<br />
<strong>in</strong>clud<strong>in</strong>g the controll<strong>in</strong>g and accreditation of the system. In this context, every<br />
<strong>in</strong>stitution is required to ma<strong>in</strong>ta<strong>in</strong> <strong>in</strong>ternal quality system <strong>in</strong> order to develop<br />
services cont<strong>in</strong>uously and to prevent possible mistakes. At the same time, the<br />
external system ensures the proper licens<strong>in</strong>g of the providers, which <strong>in</strong>cludes<br />
determ<strong>in</strong><strong>in</strong>g, ma<strong>in</strong>ta<strong>in</strong><strong>in</strong>g and publish<strong>in</strong>g standards, as well as the supervision<br />
of providers <strong>in</strong> this respect. The latter task is the responsibility of the NPHMOS.<br />
On a voluntary basis, the providers can seek additional assessment of the quality<br />
of their bus<strong>in</strong>ess operations by professional companies to obta<strong>in</strong> different<br />
certificates or they can participate <strong>in</strong> peer reviews.<br />
Earlier, the requirements of the <strong>in</strong>ternal quality assurance systems were<br />
clarified <strong>in</strong> a guidel<strong>in</strong>e published by the M<strong>in</strong>istry of <strong>Health</strong>; 7 however, the<br />
guidel<strong>in</strong>e was not updated after its expiry <strong>in</strong> 2005, although the M<strong>in</strong>istry<br />
of <strong>Health</strong> should have reviewed it every two years. Also, an overall quality<br />
monitor<strong>in</strong>g system has yet to be <strong>in</strong>troduced. For its part, the NPHMOS is<br />
charged with check<strong>in</strong>g the existence of the <strong>in</strong>ternal quality assurance system<br />
of the providers and also other related activities regard<strong>in</strong>g quality policies; these<br />
tasks are carried out via the National Centre for <strong>Health</strong>care Audit and Inspection.<br />
Based on its f<strong>in</strong>d<strong>in</strong>gs, it proposes only recommendations to ref<strong>in</strong>e the national<br />
standards published by the M<strong>in</strong>istry of <strong>Health</strong> (M<strong>in</strong>istry of <strong>Health</strong>, 2007).<br />
In 2007 more than half of all Hungarian hospitals had some k<strong>in</strong>d of<br />
certified quality assurance system, while most of these apply the International<br />
<strong>Organization</strong> for Standardization (ISO) 9000 quality system and other<br />
operational standards (Legido-Quigley et al., 2008). However, there is still<br />
no overall consolidated strategic plan for develop<strong>in</strong>g quality <strong>in</strong> health care<br />
(Gődény & Csath, 2008).<br />
There is elaborate legislation regard<strong>in</strong>g medical negligence; the necessary<br />
procedures are carried out by the National Ethics Council and its county<br />
branches (1997/20), as well as the professional chambers <strong>in</strong> cases related to<br />
their members (2006/7) s<strong>in</strong>ce 2006. For its part, the NPHMOS ensures the<br />
adm<strong>in</strong>istrative support for the councils. In certa<strong>in</strong> cases, the provider <strong>in</strong>stitutions<br />
have to apply Labour Code provisions regard<strong>in</strong>g discipl<strong>in</strong>e. A professionalethical<br />
procedure follows, dur<strong>in</strong>g which councils or the chambers exam<strong>in</strong>e<br />
negligence cases and apply discipl<strong>in</strong>ary measures. In many cases, the councils<br />
and chambers can start ethical/professional procedures when they are <strong>in</strong>formed<br />
about a certa<strong>in</strong> negligence of the professional staff (1997/20, 2006/7). All health<br />
