Day Break III Respite & Caregiver Support Services provides a comprehensive array of respite and support services to family caregivers and dependent seniors in Sunnyvale that includes statelicensed
adult day care, in-home respite, escorted transportation, caregiver support groups/education, and information and referral services.
Project Evaluation and Analysis
Identified as a Council Priority within the adopted human services priorities of the Consolidated Plan, the agency will address the need for more accessible and affordable supportive services for
frail, homebound, and dependent seniors and family caregivers in North Santa Clara County.
Abilities United will provide services to a minimum of 105 unduplicated Sunnyvale residents through the following activities: Adult Day Care, In-home respite, Escorted Transportation, Caregiver
Support Groups/Education, and Information and Referral Services.
