Figure 1.2-1 – Pathology <strong>of</strong> Developmental Defects <strong>of</strong> Enamel (DDE) from Cutress 11 DDE are generally classified into three distinct categories, namely diffuse opacities, demarcated opacities, and hypoplastic defects. 12,13 Naturally, these diffuse opacities, demarcated opacities and enamel hypoplasia are key elements <strong>of</strong> indices used to record enamel defects for epidemiological investigations. 12 The average dental practitioner may not have a full understanding <strong>of</strong> the nuances <strong>of</strong> these classifications systems as reviewing the existing indices for DDE is not part <strong>of</strong> the undergraduate dental curriculum. While enamel hypoplasia is likely the most recognized defect <strong>of</strong> enamel in both the primary and permanent dentitions it should not overshadow the significance <strong>of</strong> opacities. All three development defects <strong>of</strong> enamel signify some sort <strong>of</strong> influence on the developing ameloblast or matrix at the time <strong>of</strong> enamel development. Thus, hypoplastic enamel and enamel opacities can shed light on their possible etiologies as they 1.2-2
correspond to the timing <strong>of</strong> tooth development. This may assist in narrowing down the potential contributing agents or conditions that may predispose such enamel defects. Electron microscopy <strong>of</strong> exfoliated teeth may serve as a unique method to detect DDE at the microscopic level. 14 Current epidemiological survey techniques do not allow for further microscopic examination <strong>of</strong> the enamel surface <strong>of</strong> primary teeth. Therefore, minute areas <strong>of</strong> enamel hypoplasia may not be visible without magnification. The introduction <strong>of</strong> several indices for DDE has helped propel this area forward to the stage where established criteria are used for research purposes. Having standardized indices improves the reliability <strong>of</strong> studies and assists in the comparison <strong>of</strong> prevalence and risk factors for DDE among published reports. By definition, demarcated opacities are alterations in the normal translucency <strong>of</strong> enamel, but the thickness <strong>of</strong> enamel is not altered. 12,13,15 The defect has clear margins and may be white, cream, yellow, or brown in colour. Diffuse opacities are also changes in the translucency <strong>of</strong> enamel but these defects lack clear boundaries and can be patchy, confluent, or even linear. 12,13,15 As dental fluorosis primarily manisfests as diffuse opacities, it too is recognized to be an enamel defect. Separate indices for fluorosis exist, but their discussion is beyond the scope <strong>of</strong> this review. While opacities are qualitative changes, enamel hypoplasia involves quantitative changes to enamel. 12,13,15,16 Hypoplastic lesions may typically appear as pits, grooves, or missing enamel <strong>of</strong> varying size. 12,17 Table 1.2-1 highlights the classifications <strong>of</strong> DDE according to the modified DDE Index. 12 In some instances, both enamel hypoplasia and opacities can exist simultaneously and current DDE indices account for this possibility. 12 1.2-3
