and case records should be countersigned not less that once a<br />

fortnight.<br />

8.4.4 The keyworker should keep regular contact with other relevant<br />

workers within the Department and from other Agencies, and ensure<br />

that there are agreed arrangements for the passing of information<br />

and the planning of contact in line with the recommendations of the<br />

Case Conference.<br />

8.4.5 Fortnightly <strong>Child</strong> ‘at Risk’ Progress Report forms should be<br />

completed by the keyworker and passed to the Team Leader for<br />

countersigning and transmission to Service Unit Manager (<strong>Child</strong>ren<br />

and Families). These forms should be completed by the keyworker<br />

within three working days of the last contact with the family.<br />

8.4.6 If any significant changes occur e,g, the arrival of a new co-habitee<br />

or if a major change in the <strong>Child</strong> <strong>Protection</strong> Plan is being considered,<br />

the Team Leader should be notified immediately.<br />

8.5 Referral of Allegation of Sexual Abuse<br />

8.5.1 <strong>Child</strong> Sexual Abuse is one of the categories of <strong>Child</strong> abuse but has<br />

features which require separate consideration. The notes of<br />

procedure should be followed in conjunction with the procedures<br />

outlined earlier in this section.<br />

8.5.2 Sexual Abuse can vary from obvious abuse with a clear allegation<br />

and forensic evidence to a suspicion based on intangible but<br />

informed concern.<br />

8.5.3 <strong>Child</strong> Sexual Abuse does not necessarily call for an immediate<br />

emergency response or removal of a child from home. After the<br />

referrer has been interviewed, we must consult with other<br />

professionals within twenty four hours unless there is an immediate<br />

significant risk to the health and welfare of the child.<br />

8.5.4 If there is only suspicion that sexual abuse may have taken place<br />

then after necessary consultation with other professionals and the<br />

Team Leader a decision may be taken not to take immediate action<br />

but to convene a Case Discussion prior to making contact with the<br />

family. This decision must be endorsed by the Service Unit Manager<br />

(<strong>Child</strong>ren and Families).<br />

8.5.5 The Case Discussion is a formal mechanism which allows<br />

professionals to consult with each other, co-ordinate information<br />

already known about the child and family and plan how best to<br />

proceed. If a Case Discussion is to be held it must be convened<br />

within five working days of the initial referral being made. It must be<br />

chaired by the Team Leader/Service Unit Manager (<strong>Child</strong>ren and<br />
