Note: The type of unit must be indicated. The organization, or facility detected. If it cannot<br />

be identified without a doubt, describe in as much detail as possible, including uni<strong>for</strong>ms,<br />

vehicle markings, and other identifying in<strong>for</strong>mation.<br />

Note: The equipment type or types, and amplifying sub-type if applicable, must be<br />

indicated. Add a narrative if necessary to clarify, describe, or explain the type of equipment.<br />

The nomenclature, type, and quantity of all equipment observed must be provided. If<br />

equipment cannot be identified without a doubt, describe in as much detail as possible.<br />

i. LINE 9 – assessment (apparent reason <strong>for</strong> or purpose of the activity<br />

observed, and apparent threats to or opportunities <strong>for</strong> friendly <strong>for</strong>ces).<br />

j. LINE 10 –narrative (free text <strong>for</strong> clarifying report).<br />

Note: The actions that you have taken that are related to the detected activity: attack,<br />

withdraw, continue to observe, or other. When feasible, state potential <strong>for</strong> subsequent<br />

reports such as air support request, battle damage assessment (BDA) report, call <strong>for</strong> fire,<br />

casualty report, explosive ordinance disposal (EOD) support, or medical evacuation<br />

(MEDEVAC).<br />

k. LINE 11 – authentication (report authentication) per SOP.<br />
