Access <strong>for</strong> Work on or Near High Voltage ElectricalApparatusstatements between the authorised <strong>electrical</strong>operator and the recipient in charge beingconfirmed by a witness at each end.9.2.5.3 At the time <strong>of</strong> issue the authorised <strong>electrical</strong>operator shall describe and, where practicable,show the <strong>electrical</strong> apparatus covered by the<strong>electrical</strong> access permit and the precautions taken,to the recipient in charge and all the initialrecipients.The authorised <strong>electrical</strong> operator shall alsodescribe or point out the nearest points <strong>of</strong> supplyand adjacent live <strong>electrical</strong> apparatus. In the case<strong>of</strong> an <strong>electrical</strong> access permit issued by telephoneor radio the recipient in charge shall assume theseresponsibilities.9.2.5.4 Each <strong>electrical</strong> access permit shall beissued to a recipient in charge. The recipient incharge shall ensure that all members <strong>of</strong> the <strong>work</strong>party who will approach the <strong>electrical</strong> apparatussign on the <strong>electrical</strong> access permit.9.2.5.5 The issuer and authorised recipients allhave a responsibility to ensure that the condition <strong>of</strong>the <strong>electrical</strong> apparatus, covered by the <strong>electrical</strong>access permit, is such that it shall be safe <strong>for</strong> theproposed <strong>work</strong> to be undertaken.Blue Book – December 2005 Page 81 <strong>of</strong> 148
