The gaps <strong>in</strong> develop<strong>in</strong>g the role of the <strong>patient</strong>/consumer <strong>in</strong> <strong>patient</strong> <strong>safety</strong> are visible <strong>in</strong>:<br />
» the current organization of <strong>care</strong> (<strong>in</strong>sufficiently transparent and reliable);<br />
» lack of <strong>in</strong>sight <strong>in</strong>to the values, knowledge, attitudes, needs, ideas and read<strong>in</strong>ess of<br />
<strong>patient</strong>s/consumers to play a role <strong>in</strong> <strong>patient</strong> <strong>safety</strong>; and<br />
» <strong>in</strong>sufficient attention be<strong>in</strong>g given to the communication and <strong>health</strong> literacy skills of<br />
professionals and <strong>patient</strong>s/consumers.<br />
Motivat<strong>in</strong>g professionals is a guid<strong>in</strong>g pr<strong>in</strong>ciple for support<strong>in</strong>g and encourag<strong>in</strong>g <strong>patient</strong>s/<br />
consumers to play a role <strong>in</strong> <strong>patient</strong> <strong>safety</strong>.<br />
Conclusion<br />
The relationship between the <strong>health</strong> <strong>care</strong> professional and the <strong>patient</strong>/consumer is the<br />
key to develop<strong>in</strong>g the role of the latter <strong>in</strong> <strong>patient</strong> <strong>safety</strong>. The first prerequisite is shared<br />
awareness of the fact that a role for the <strong>patient</strong>/consumer is possible, accompanied by the<br />
motivation and skills to enable the activation of this role. A number of themes affect the<br />
relationship between the professional and the <strong>patient</strong>/consumer and are important for<br />
develop<strong>in</strong>g the <strong>in</strong>tegrated approach required:<br />
» professionalization and facilitation of <strong>patient</strong>/consumer collectives (councils and<br />
representatives) so that they can fulfil a role <strong>in</strong> <strong>care</strong> policy and primary <strong>care</strong> process<br />
redesign;<br />
» research <strong>in</strong>to the effect and importance of, and the <strong>in</strong>teraction between, the five<br />
context factors on which the fulfilment of the role of the <strong>patient</strong>/consumer depends:<br />
1. the specific <strong>patient</strong> (knowledge and op<strong>in</strong>ions)<br />
2. the illness (phase and characteristics)<br />
3. the professional (knowledge, op<strong>in</strong>ions and “<strong>in</strong>vit<strong>in</strong>g” behaviour)<br />
4. the sett<strong>in</strong>g<br />
5. the type of <strong>safety</strong> behaviour the client exhibits;<br />
» research <strong>in</strong>to exist<strong>in</strong>g <strong>in</strong>itiatives (<strong>in</strong>terventions) <strong>in</strong> terms of their effects, <strong>in</strong>clud<strong>in</strong>g<br />
development, <strong>in</strong>novation and extend<strong>in</strong>g them to other sett<strong>in</strong>gs;<br />
» develop<strong>in</strong>g a national <strong>in</strong>formation and knowledge centre for all parties <strong>in</strong>volved <strong>in</strong><br />
<strong>health</strong> <strong>care</strong>, with public <strong>in</strong>formation, tools and education packages to give substance<br />
to the role of the <strong>patient</strong>/consumer <strong>in</strong> <strong>patient</strong> <strong>safety</strong>;<br />
» sett<strong>in</strong>g up a tra<strong>in</strong><strong>in</strong>g programme (such as a <strong>patient</strong> <strong>safety</strong> officer programme) for<br />
<strong>health</strong> <strong>care</strong> organizations, focus<strong>in</strong>g on leadership, culture and structure;<br />
» evaluat<strong>in</strong>g and optimiz<strong>in</strong>g the legislation and regulations relat<strong>in</strong>g to the role of the<br />
<strong>patient</strong>/consumer; and<br />
» <strong>in</strong>volv<strong>in</strong>g the <strong>in</strong>sight of the client <strong>in</strong> guidel<strong>in</strong>es, <strong>in</strong>dicators and standards of <strong>care</strong> and<br />
mak<strong>in</strong>g <strong>health</strong> <strong>care</strong> options clear and understandable to the <strong>patient</strong>/consumer.<br />
In the long term, it is essential that <strong>patient</strong>s’ perspectives be <strong>in</strong>corporated at four levels:<br />
1. the <strong>in</strong>dividual <strong>care</strong> process and organizations<br />
2. the national <strong>health</strong> <strong>care</strong> organizations<br />
3. the national <strong>health</strong> <strong>care</strong> system<br />
4. laws and regulations.
