awareness of the sociomedical burden of HAIs and implement<strong>in</strong>g target-oriented<br />

<strong>in</strong>fection control <strong>in</strong>terventions (5). The survey also supported the decision-mak<strong>in</strong>g<br />

process for improv<strong>in</strong>g <strong>in</strong>fection control and optimiz<strong>in</strong>g hospital <strong>in</strong>frastructure and<br />

provided a valuable <strong>in</strong>strument to support ongo<strong>in</strong>g surveillance and control systems.<br />

Patient <strong>participation</strong> <strong>in</strong> hand hygiene <strong>in</strong> Bulgarian <strong>health</strong> <strong>care</strong><br />

Total HAI <strong>in</strong>cidence reduced from 12.7 per 1000 discharged <strong>patient</strong>s <strong>in</strong> 2007 to 11.2<br />

<strong>in</strong> 2008 and 10.2 <strong>in</strong> 2009. Recorded <strong>in</strong>cidence decreased significantly (about 10%)<br />

compared to the late 1990s. Morbidity <strong>in</strong> university hospitals decl<strong>in</strong>ed from 13.7 per<br />

1000 discharged <strong>patient</strong>s <strong>in</strong> 2003 to 10.7 <strong>in</strong> 2009 and from 11 to 9 <strong>in</strong> primary municipal<br />

hospitals. In secondary hospitals, where morbidity varies more widely, a substantial<br />

decl<strong>in</strong>e from 13 to 8 per 1000 discharged <strong>patient</strong>s was recorded (7).<br />

Prevent<strong>in</strong>g HAI was identified as a fundamental priority and the core topic of the First<br />

Global Patient Safety Challenge, “Clean Care is Safer Care” (8−10). Implementation<br />

strategies <strong>in</strong>clude <strong>in</strong>tegrat<strong>in</strong>g multiple <strong>in</strong>terventions <strong>related</strong> to blood, <strong>in</strong>jection and<br />

cl<strong>in</strong>ical procedure <strong>safety</strong>, water, sanitation and waste management, and promot<strong>in</strong>g hand<br />

hygiene as a cornerstone of safe <strong>health</strong> <strong>care</strong>.<br />

Bulgarian legal and regulatory framework on prevent<strong>in</strong>g HAI and <strong>in</strong>volv<strong>in</strong>g<br />

<strong>patient</strong>s<br />

The legislative framework cover<strong>in</strong>g this topic comprises several documents, described <strong>in</strong><br />

this section.<br />

The Law on Health of 10 August 2004 (State Gazette (SG) No. 70 of 10 August<br />

2004; updated by SG No. 54 of 17 July 2012), effective from 1 January 2005, regulates<br />

dedicated activities and quality control measures relat<strong>in</strong>g to <strong>health</strong> <strong>care</strong>, consequently<br />

cover<strong>in</strong>g <strong>patient</strong> <strong>safety</strong> aspects. The act regulates <strong>patient</strong>s’ rights and obligations <strong>in</strong><br />

accordance with the Council of Europe’s biomedic<strong>in</strong>e convention of 1997 (11).<br />

The ma<strong>in</strong> law on <strong>in</strong>fection control organization is the Ord<strong>in</strong>ance of the M<strong>in</strong>istry of<br />

Health No. 13 on Organization of Hospital Infections Prevention and Control (SG No.<br />

95/1998r.), issued <strong>in</strong> 1998 and updated <strong>in</strong> 2005 by M<strong>in</strong>isterial Ord<strong>in</strong>ance No. 2 of 10<br />

January 2005 on the Organization of Prevention and Control of Nosocomial Infections<br />

(SG No. 8 of 21 January 2005).<br />

The National Medical Standard on Prevention and Control of Health<strong>care</strong> Associated<br />

Infections was endorsed by the M<strong>in</strong>istry of Health with Ord<strong>in</strong>ance No. 39 of 26 August<br />

2010 (SG No. 69 of 3 September 2010). It <strong>in</strong>cludes chapters on HAI prevention,<br />

hand hygiene, hand wash<strong>in</strong>g and hand dis<strong>in</strong>fection. This act discusses the <strong>in</strong>dications,<br />

elements and techniques of hand hygiene.<br />

The National Reference Centre for Nosocomial Infections (NRC−HAI) was set up<br />

<strong>in</strong> 2007 to support the prevention and control of HAIs. This is a specialized unit at<br />

the National Centre for Infectious and Parasitic Diseases (NCIPD) established with<br />

support from the M<strong>in</strong>istry of Health and Swiss Red Cross via the hospital hygiene<br />

programme. NCIPD is the competent and authorized local organization for HAI<br />

prevention. An expert council for <strong>in</strong>fection control has been created as an advisory<br />
