Withdrawal<br />
While enrolled in the College <strong>of</strong> Nursing, you may need to interrupt your program for various<br />
reasons. The <strong>University</strong> <strong>of</strong> <strong>Kentucky</strong> Bulletin covers university withdrawal policy.<br />
You must be cleared by the College and the UK Registrar to maintain the integrity <strong>of</strong> your<br />
academic record.<br />
• Request College clearance: Ask your faculty advisor or Office <strong>of</strong> <strong>Student</strong> Services for<br />
a College Withdrawal Form. Obtain your faculty advisor's signature and return the<br />
form to the appropriate Assistant Dean.<br />
• Request Registrar clearance: Go to the <strong>University</strong> Registrar's Office, room 10<br />
Funkhouser Building or fax a letter signed and addressed to the UK Registrar, indicating<br />
your decision to withdraw. Include your full name, social security number, and<br />
remember to sign your name. Fax the letter to (859) 257-7160.<br />
