Instructions for Scheduling Final Exam (Ph.D.) - University of Kentucky College of Nurs... http://www.mc.uky.edu/nursing/StuResources/grads/handbook2005-06/schedule_exam.htm Page 1 of 2 2/24/2011 Instructions for Scheduling Final Exam (Ph.D.) The scheduling of a doctoral final examination involves three steps (see Graduate School Bulletin): Step 1 The first step is the submission of the Notification of Intent to Schedule a Final Doctoral Examination. The assistant dean for research and Ph.D. program or graduate student affairs officer will complete this form when advised by you or your committee chair. The form must be delivered to the Graduate School a minimum of eight weeks prior to the anticipated defense week. The submission of this form triggers two processes in the Graduate School. First, the student's record is reviewed to determine if there are any outstanding deficiencies ("I" or "S" grades, satisfaction of residency requirements, time limits, etc.), and the student and assistant dean for research and Ph.D. program are notified of any such issues. Second, the Graduate School identifies and assigns an appropriate outside examiner for the final examination. In order to prepare the Notification of Intent form we need the following information from the student: (1) dissertation title, (2) dissertation abstract, (3) target defense date – provide a 1-2 week window, and (4) suggested department from which the outside examiner could be selected. NOTE: Include the assistant dean for research and Ph.D. program when scheduling your defense. Also provide the assistant dean a draft of your dissertation at the same point you provide it to your outside examiner. Step 2 The second step is the submission to the Graduate School of the Request for Final Doctoral Examination, which is the form that specifies the date, time, and location of the examination. This form must be submitted at least two weeks prior to the exam, and must be accompanied by the signed Dissertation Approval sheet. At this point in the process you must also deliver the draft dissertation to your outside examiner. The two-week lead time associated with this step is intended to accomplish the following: Step 3 • It allows for sufficient notice to be provided to the University community regarding the final doctoral examination, which is a public event open to all members of the University community. • It insures that the outside examiner is provided with a finished copy of the dissertation at least two weeks prior to the exam, allowing sufficient time for review. Following the final exam (oral defense) students must submit two original unbound dissertation documents to the UK Graduate School in accordance with the Graduate School

See www.research.uky.edu/gs/thesdissprep.html Students must schedule an appointment with the Graduate School at (859) 257-4613 to have their document formatting checked and the dissertation accepted. Students are encouraged to have the Graduate School check the formatting on a draft copy in advance of printing the two final copies on the more expensive acid-free paper. The final copy of the dissertation must be presented to the Graduate School within 60 days of the final exam or on the last day to present a dissertation for a graduating candidate, if a specific graduation date is required (i.e. a student defending in mid- April who wants a May degree must submit their document in final form by the deadline for that term, turnaround is much less than 60 days). Please contact the College of Nursing graduate student affairs officer or assistant dean for research and Ph.D. program with any questions or concerns regarding these deadlines and policies. These deadlines are not flexible. Paperwork may be submitted in advance of these deadlines. In addition, you must meet all Graduate School requirements for format as specified in Instructions for Preparation of Dissertations and Doctor of Musical Arts Projects.
