Health Fee and Activity Fees Information<br />

<strong>Student</strong> Information<br />

Full-time students taking classes on the Lexington campus are required to pay health and<br />

activity fees each semester. However, part-time students and students registered in NUR 748,<br />

749, or 769 for 0 credit hours may opt to voluntarily pay these fees.<br />

Part-time students and students registered in NUR 748, 749, 767, or 769 for 0 credit hours may<br />

opt to pay only the health fee or the health fee and Seaton Center fee. Paying the Seaton Center<br />

fee without the health fee is not an option and paying a portion <strong>of</strong> the Seaton Center fee is not<br />

an option. Paying these fees entitles students to utilize <strong>University</strong> resources and receive<br />

medical treatment by the <strong>University</strong> <strong>Student</strong> Health Services.<br />

How part-time students and students registered in NUR 748, 749, 767, or 769 for 0 credit<br />

hours may pay the voluntary health fee and Seaton Center fees for the fall and spring<br />

semesters:<br />

18 Funkhouser Building, 8:30am – 4pm., Monday through Friday.<br />

<strong>Student</strong> Billing Services mailing address is PO Box 931147, Cleveland, Ohio 44193. When<br />

mailing a check, please provide a social security number or student ID number, the semester<br />

and “health fee” or “health and activity fees” should appear on the check.<br />

If you prefer, you may also take a check in-person to the 18 Funkhouser Building.<br />

These fees must be paid by the first day <strong>of</strong> classes in the fall and spring semesters.<br />

How part-time students or students registered in NUR 748, 749, 767, or 769 for 0 credit<br />

hours may pay the voluntary health fee for the summer:<br />

The health fee may be voluntarily paid for health care during the summer. In order to be<br />

eligible for services in the summer, the fee must be paid by the first day <strong>of</strong> classes for students<br />

enrolled in the four-week intersession or the eight-week summer session. <strong>Student</strong>s pay in<br />

<strong>Student</strong> Billing Services, 18 Funkhouser. Continuing students enrolled in the spring and<br />

priority registered for the fall must go to the Health Service, B163 <strong>Kentucky</strong> Clinic, to have<br />

their eligibility to purchase the fee verified, then go to <strong>Student</strong> Billing Services, 18<br />

Funkhouser, to pay the fee. A social security number or student ID number, "summer", and<br />

"health fee" or "health and activity fees" should appear on the check.<br />
