and Urine Creatinine<br />

Background: Measurement of serum<br />
creatinine in newborns can be<br />
problematic for several reas<strong>on</strong>s. In<br />
infants born with very low birth weight,<br />
volume loss <str<strong>on</strong>g>from</str<strong>on</strong>g> blood draws can<br />
account for significant blood loss<br />
limiting clinical and research<br />
measurements. In additi<strong>on</strong>, bilirubin<br />
(known to be high in ne<strong>on</strong>ates) and<br />
medicati<strong>on</strong>s can interfere with creatinine<br />
measurement by the Jaffe reacti<strong>on</strong>.<br />
Purpose: We evaluated the differences<br />
in laboratory sampling between different<br />
methodologies (Mass Spectrometry<br />
(MS) vs. Jaffe). In additi<strong>on</strong>, we<br />
evaluated the ability to reliably replicate<br />
results with variati<strong>on</strong>s in time <str<strong>on</strong>g>from</str<strong>on</strong>g> lab<br />
draw to sample measurement [immediate<br />
(within 2 hours), at 24 hours, and at 3<br />
days). Finally we assessed how<br />
measurements may be influenced by the<br />
type of storage (Refrigerati<strong>on</strong> -4 degree<br />
C vs. Freezer -8 degree C). Methods:<br />
We performed a prospective laboratory<br />
analysis using whole blood and fresh<br />
voided urine <str<strong>on</strong>g>from</str<strong>on</strong>g> 6 healthy adults who<br />
had neither kidney or liver disease, and<br />
were not taking medicati<strong>on</strong>s known to<br />
interfere with creatinine determinati<strong>on</strong>.<br />
Each samples was processed using <strong>on</strong>ly<br />
2 mcl of sample. The first method (MS)<br />
involved tandem Mass Spectrometry<br />
using multiple reacti<strong>on</strong> m<strong>on</strong>itoring and<br />
quantitated via stable isotope diluti<strong>on</strong>.<br />
The sec<strong>on</strong>d used Jaffe methodology <strong>on</strong> a<br />
Beckman machine. Results: There was a<br />
very high correlati<strong>on</strong> (r= .996) between<br />
MS and Jaffe samples and a mean %<br />
differences between samples of 5.15 +<br />
23.49 ng/dl. There was good correlati<strong>on</strong>s<br />
and limited % difference between<br />
measurement times and storage type in<br />
both MS and Jaffee. (Table 1).<br />

C<strong>on</strong>clusi<strong>on</strong>s: Although variati<strong>on</strong>s exist<br />
between creatinine measurements using<br />
Jaffe and Mass Spectrometry there is a<br />
high degree of correlati<strong>on</strong> even when<br />
performing these tests using<br />
microsamples. Samples were relatively<br />
unaffected after 24 hours in a<br />
refrigerator, freezing for 1 day or<br />
freezing for 3 days.<br />
