Assessing the Microcirculati<strong>on</strong><br />

Educati<strong>on</strong>al Objectives:<br />

1. Discuss the different methods for evaluating the microcirculati<strong>on</strong><br />

2. To review the different scores available for microcirculati<strong>on</strong> analysis, and which measurements should be included.<br />

3. Discuss how m<strong>on</strong>itoring of the microcirculati<strong>on</strong> may help us at the bedside<br />

C<strong>on</strong>tent Descripti<strong>on</strong>:<br />

Microvascular perfusi<strong>on</strong> plays an important role in the development of organ failure in hospitalized patients.<br />

Although microcirculati<strong>on</strong> is the primary site of oxygen and nutrient exchange, microvascular oxygen delivery<br />

cannot be predicted <str<strong>on</strong>g>from</str<strong>on</strong>g> global hemodynamic measurements. Hemodynamic assessment has been limited to<br />

measurements of cardiac output and oxygen delivery, which are indirect measurements of microcirculati<strong>on</strong>.<br />

However, several observati<strong>on</strong>al studies and randomized c<strong>on</strong>trolled trials trying to improve these parameters based<br />

<strong>on</strong> informati<strong>on</strong> derived <str<strong>on</strong>g>from</str<strong>on</strong>g> pulm<strong>on</strong>ary artery catheter have shown no benefits in outcomes.<br />

In critically-ill patients improvement in microvascular perfusi<strong>on</strong> should be to <strong>on</strong>e of the major therapeutic goals.<br />

Recent advances in technology have allowed direct visualizati<strong>on</strong> of the microcirculati<strong>on</strong>. The octag<strong>on</strong>al polarizati<strong>on</strong><br />

spectral (OPS) and the side stream dark field (SDF) imaging devices provide high c<strong>on</strong>trast images of the<br />

microvasculature. Experimental studies have shown alterati<strong>on</strong>s in the microcirculati<strong>on</strong> comm<strong>on</strong> to different pathological<br />

process. OPS and SDF imaging have been used to assess microvascular alterati<strong>on</strong>s in many disease<br />

processes such as: sepsis, painful crises in sickle cell disease, bacterial infecti<strong>on</strong> in cirrhosis, leukostasis in patients<br />

with chr<strong>on</strong>ic myeloid leukemia, and changes in the volume status of hemodialysis patient. It has also been used to<br />

assess the effect of different therapies <strong>on</strong> microcirculati<strong>on</strong> in experimental animal models and in septic patients.<br />

Additi<strong>on</strong>ally, in critically-ill patients with sepsis and septic shock, recent studies have correlated microcirculatory<br />

flow abnormalities with hemodynamics, oxygen transport and survival.<br />

The introducti<strong>on</strong> of OPS and SDF imaging has opened challenging new perspectives in in vivo research of microcirculatory<br />

alterati<strong>on</strong>s, and has highlighted the importance of microcirculatory alterati<strong>on</strong>s in multiple organ failure and sepsis.<br />
