Educati<strong>on</strong>al Objectives:<br />

• Discuss the physiology of the microcirculati<strong>on</strong> and the parameters which defines its functi<strong>on</strong> in different<br />

organs. Discuss the physiological determinants of perfusi<strong>on</strong> and oxygenati<strong>on</strong> of the microcirculati<strong>on</strong> and how<br />

these can be measured in the clinical scenario<br />

• Discuss the latest developments in the bed side assessment of the microcirculati<strong>on</strong> and their meaning in terms<br />

of prognosis and resp<strong>on</strong>se to therapy.<br />

• Practical dem<strong>on</strong>strati<strong>on</strong> of the use of sublingual SDF imaging<br />

C<strong>on</strong>tent Descripti<strong>on</strong>:<br />

In this lecture we will discuss the (patho)physiological significance of microcirculatory alterati<strong>on</strong>s in clinical<br />

states of sepsis, shock and resuscitati<strong>on</strong> and how its m<strong>on</strong>itoring at the bed side has played a central role in identifying<br />

its significance. Besides the clinical background of microcirculatory , the methods both in terms of hardware<br />

and analysis methods will be discussed. Following this recent improvements in the techniques of m<strong>on</strong>itoring<br />

and interpretati<strong>on</strong> of microcirculatory alterati<strong>on</strong>s in critical illness as measured by direct sublingual microcirculati<strong>on</strong><br />

observati<strong>on</strong>. We will present our new developed software which gives instant evaluati<strong>on</strong> of the images<br />

as well as improvements in the devices for reducing pressure artefacts. A new improved imaging device will be<br />

presented. Next a new internet platform will be presented for allowing internati<strong>on</strong>al exchange of images and setting<br />

the scene for multi-central interventi<strong>on</strong> trials with as goal the improvement of the microcirculati<strong>on</strong> to be<br />

held. In this c<strong>on</strong>text the progress of epidemiological survey of microcirculatory alterati<strong>on</strong>s (perfusi<strong>on</strong> as well<br />

oxygenati<strong>on</strong>) in the critically ill patients will be presented.<br />
