State of the art; <strong>Medical</strong> <strong>Balneology</strong> in Turkey<br />

Health resort medicine or spa medicine has a great tradition in Turkey. This tradition<br />

is still vivid <strong>and</strong> mostly accompanied by empiricism <strong>and</strong> intuitiveness. We<br />

have collected data on the characteristics of traditional <strong>and</strong> modern balneological<br />

<strong>and</strong> spa therapy forms in Turkey that are widely used in the treatment of rheumatic<br />

diseases. Spa <strong>and</strong> balneotherapy forms differ according to the spa centre; they may<br />

be either stationary or ambulatory; <strong>and</strong> mono balneotherapy or complex spa therapy<br />

modalities. The main indication for spa therapy <strong>and</strong> balneotherapy in Turkey is<br />

rheumatic diseases. Unlike other European countries, nearly 90% Turkish patients<br />

who undergo balneotherapy or spa therapy courses are suffering from rheumatic<br />

disorders, mostly degenerative joint diseases, often soft tissue <strong>and</strong> rarely inflammatory<br />

rheumatic diseases.<br />

The recent balneological research in Turkey that is being carried out mainly in<br />

two university units; Department of <strong>Medical</strong> Ecology <strong>and</strong> Hydroclimatology of<br />

Istanbul <strong>Medical</strong> Faculty of Istanbul University <strong>and</strong> Atatürk Balneotherapy <strong>and</strong><br />

Rehabilitation Center of Uludağ University in Bursa. The other two departments of<br />

<strong>Medical</strong> Ecology <strong>and</strong> Hydroclimatology in Ankara <strong>and</strong> Tokat <strong>and</strong> the Departments<br />

of Physical Medicine <strong>and</strong> Rehabilitation in Afyon <strong>and</strong> Denizli also appeared with<br />

their research activities in recent years. While as the education <strong>and</strong> training centers<br />

the <strong>Medical</strong> Ecology <strong>and</strong> Hydroclimatology departments are concentrated in the<br />

research on a wide range of scientific topics in balneology such as experimental<br />

studies, mechanisms of effect studies, effects of different spa packages, effects of<br />

bathing <strong>and</strong>/or drinking cures <strong>and</strong> specific effects of Sulfur, CO2 <strong>and</strong> NaCl (salty<br />

<strong>and</strong> brine) waters, as a University Balneological research center, Atatürk Balneotherapy<br />

<strong>and</strong> Rehabilitation Center of Uludağ University in Bursa is being conducting<br />

the relevant r<strong>and</strong>omized controlled trials evaluating the effects of balneotherapy<br />

in majority of rheumatic conditions including rheumatoid arthritis <strong>and</strong> ankylosing<br />

spondylitis. It is interesting to note that balneological trials <strong>and</strong> research have been<br />

increasingly published lately in English. This phenomenon not only reflects the<br />

willingness of the authors sharing their evidence in international scientific arena<br />

with international experts but also helps to improve the quality of research methodology<br />

in medical balneolgy <strong>and</strong> hydroclimatology in Turkey which is not an internal<br />

challenge, rather an international one.<br />
