3.7.2 Posted Materials<br />

c) CCM will act as <strong>the</strong> Civil Rights Liaison (CRL)<br />

between <strong>the</strong> Contracted agency and <strong>the</strong> CCA and<br />

Civil Rights and Customer Relations (CRCR)<br />

Section of DPSS.<br />

d) All CCM/CRLs must <strong>for</strong>ward all PA 607s to <strong>the</strong> CCA<br />

within two (2) business days.<br />

e) CCM/CRLs should not attempt to investigate Civil<br />

Rights complaints. Investigations are handled by<br />

CRCR and shall adhere to <strong>the</strong> Civil Rights<br />

Complaint Flowchart Contractor Process, (Contract,<br />

Attachment U.)<br />

Contractor shall post in each Contractor’s facility (where <strong>the</strong>y are<br />

easily accessible to employees) Equal Employment Opportunity<br />

(EEO) and State-approved Nondiscrimination in <strong>Services</strong> notices.<br />

Contractor shall ensure that <strong>the</strong> Contractor Employee<br />

Acknowledgement and Confidentiality Agreement, Attachments I-1<br />

and I-2 are signed and copies on file <strong>for</strong> each Contract employee<br />

prior to employee starting work under this contract.<br />

The percentage of <strong>the</strong> number of Contractor staff who leave <strong>the</strong> long-term<br />

contracted work <strong>for</strong>ce, to <strong>the</strong> total long-term contracted work <strong>for</strong>ce. The long-term<br />

Contracted work <strong>for</strong>ce is <strong>the</strong> highest number of positions that are filled during <strong>the</strong><br />

period under consideration. The turnover rate shall be cumulative and shall be<br />

computed each month <strong>for</strong> <strong>the</strong> term of <strong>the</strong> Contract. Computation of <strong>the</strong> turnover<br />

rate shall be projected to reflect a full year should <strong>the</strong> period be less than twelve<br />

(12) months (e.g., a turnover rate of 6 percent <strong>for</strong> 6 months would be projected to<br />

reflect an annual turnover rate of 12 percent). The following is an example of<br />

how <strong>the</strong> annual cumulative turnover rate may be calculated<br />
