CIVIL RIGHTS COMPLAINT FLOWCHART<br />

CONTRACTOR PROCESS<br />

Contractor employee receives a complaint<br />

of discrimination (written or verbal).<br />

CCM/CRL receives a complaint of<br />

discrimination (written or verbal).<br />

Contractor employee immediately provides a PA 607<br />

(in <strong>the</strong> participant’s designated/preferred language)<br />

and if requested, assists <strong>the</strong> participant in completing<br />

a PA 607.<br />

Contractor employee <strong>for</strong>wards <strong>the</strong> PA<br />

607 to <strong>the</strong> Contractor Contract Manager<br />

(CCM)/Civil Rights Liaison (CRL).<br />

CCM/CRL immediately<br />

provides a PA 607 (in <strong>the</strong><br />

participant’s<br />

designated/preferred language)<br />

and if requested, assists <strong>the</strong><br />

CCM/CRL logs <strong>the</strong> complaint on <strong>the</strong><br />

Contractor’s Civil Rights Complaint Log.<br />

Within two (2) business days, CCM/CRL<br />

<strong>for</strong>wards <strong>the</strong> PA 607 to <strong>the</strong> County Contract<br />

Administrator (CCA) and maintains a copy.<br />

The CCA immediately <strong>for</strong>wards <strong>the</strong> PA 607 to<br />

DPSS’ Civil Rights & Customer Relations<br />

Section (CRCR)and maintains a copy.<br />

Within 60 calendar days following <strong>the</strong> receipt of <strong>the</strong> complaint by<br />

CRCR, an investigation is conducted and a report of findings is<br />

<strong>for</strong>warded to <strong>the</strong> State, complainant, CCA and <strong>the</strong> CCM/CRL.<br />
