<strong>RFP</strong> APPENDIX A, SAMPLE CONTRACT<br />

STATEWIDE FINGERPRINT IMAGING SYSTEM (SFIS) TERMINAL OPERATOR SERVICES<br />

2.1 Acceptable Quality Level (AQL): A measure to express <strong>the</strong> allowable<br />

leeway or variance from <strong>the</strong> Contract standard, above which <strong>the</strong> County<br />

will reject a specific service. The AQL does not imply that it is acceptable<br />

to vary from <strong>the</strong> Standard, or that <strong>the</strong> Contractor may knowingly per<strong>for</strong>m in<br />

a defective way. The AQL recognizes <strong>the</strong> fact of unintentional human<br />

error, and that less than standard per<strong>for</strong>mance may sometimes be<br />

unintentional.<br />

2.2 Applicant: A person whose public assistance application is pending.<br />

2.3 Board of Supervisors (Board): The Los Angeles County Board of<br />

Supervisors; <strong>the</strong> governing body of <strong>the</strong> County of Los Angeles.<br />

2.4 Contractor’s Budget: The document that details <strong>the</strong> Contractor’s costs <strong>for</strong><br />

providing services and is included in <strong>the</strong> Contract. The Budget includes <strong>the</strong><br />

following:<br />

a) Direct Costs - Payroll, Employee Benefits (Medical, Dental, Life<br />

Insurance), Payroll Taxes, Insurance (Real, Personal, etc., as required<br />

by <strong>the</strong> Contract), Supplies, Applicable Taxes and o<strong>the</strong>r specified costs.<br />

b) Indirect Costs - General Accounting/Bookkeeping, Management<br />

Overhead and o<strong>the</strong>r specified costs.<br />

c) Total Cost to Contract <strong>Services</strong> - The total cost of Direct and Indirect<br />

Costs.<br />

2.5 Business Days: Business days are Monday through Friday, excluding<br />

County holidays.<br />

2.6 CalWORKs: (Cali<strong>for</strong>nia Work Opportunities and Responsibilities to<br />

Kids Program): a federally mandated public assistance program that<br />

provides financial assistance to families with dependent children.<br />

2.7 Contract: Contract executed between County and Contractor. It sets <strong>for</strong>th<br />

<strong>the</strong> terms and conditions <strong>for</strong> <strong>the</strong> issuance and per<strong>for</strong>mance of <strong>the</strong><br />

Statement of Work, Attachment A to this Contract.<br />

2.8 Contract Management Division (CMD): The Department of Public Social<br />

<strong>Services</strong>’ Division responsible <strong>for</strong> <strong>the</strong> Contract.<br />

2.9 Contractor: The sole proprietor, partnership, or corporation that has<br />

entered into this Contract with <strong>the</strong> County to per<strong>for</strong>m or execute <strong>the</strong> work<br />

covered by <strong>the</strong> Statement of Work.<br />
