<strong>The</strong> implementation <strong>of</strong> a model <strong>of</strong> person-centred practice in older person settings<br />
involved being given the opportunity to partake.<br />
• “My Day My Way “<br />
<strong>The</strong> Importance <strong>of</strong> getting to know the person not just the resident and what is<br />
important to them in their daily care is central to the concepts underpinning PCC. As<br />
part <strong>of</strong> the <strong>Person</strong>-<strong>Centred</strong> Care Programme an innovative exercise “My Day My<br />
Way” was developed by participants in BCNU (Figures 2.1 and 2.2). This getting to<br />
know me exercise can be carried out before a resident is admitted to the unit, when<br />
they are admitted as part <strong>of</strong> their admission procedure or now as part <strong>of</strong> their ongoing<br />
assessment. All staff at unit level are involved, which has greatly enhanced team<br />
work.<br />

Towards Developing <strong>Person</strong> <strong>Centred</strong> Care for Residents<br />
<strong>The</strong> Importance <strong>of</strong> getting to know the person not just the resident and what<br />
is important to them in their daily care is kernel to the concepts underpinning<br />
PCC. How can we help staff be more person-centred in their care for the<br />
individual person?<br />
As part <strong>of</strong> the <strong>Person</strong> <strong>Centred</strong> Care Programme an innovative way “My Day My<br />
Way” was developed by participants.<br />
• This getting to know me exercise can be carried out before a resident<br />
is admitted to the unit, when they are admitted as part <strong>of</strong> their<br />
admission procedure or now as part <strong>of</strong> their ongoing assessment.<br />
• This can be completed with the resident by a family member care staff<br />
named nurse.<br />
• It should be kept in the residents care plan which is accessible to all<br />
staff.<br />
• It should be reviewed and updated as part <strong>of</strong> the ongoing evaluation <strong>of</strong><br />
the residents care<br />
• All new staff should familiarise themselves with the plan<br />
