<strong>The</strong> implementation <strong>of</strong> a model <strong>of</strong> person-centred practice in older person settings<br />
Celebrating the National Programme in 2009<br />
<strong>The</strong> two sites had a celebratory event towards the end <strong>of</strong> 2009, the purpose <strong>of</strong> the<br />
event was to celebrate with residents, their families, and staff both in the hospital and<br />
in the community the achievements over the two years. Both were successful events<br />
and provided a foundation to evaluate work ongoing and plans to sustain the PD<br />
work.<br />
Sustainability <strong>of</strong> the Programme<br />
<strong>The</strong> programme group on both sites are keen to continue to work towards<br />
implementing a person-centred philosophy and plans are in place in both sites to<br />
progress this work. <strong>The</strong> increased involvement <strong>of</strong> residents/families and the wider<br />
healthcare team over the next year is part <strong>of</strong> the sustainability plan.<br />
Summary<br />
<strong>The</strong> PD programme resulted in many achievements and changes for residents and<br />
staff in both sites in the North West. <strong>The</strong>re was significant growth in the strength <strong>of</strong><br />
the group and this was demonstrated in many ways. <strong>In</strong>creased interaction between<br />
residents, families and staff has occurred due to cultural change brought about by<br />
changing the way things are done – being part <strong>of</strong> the programme has contributed<br />
directly to this. <strong>The</strong> implementation <strong>of</strong> action planning through the process <strong>of</strong><br />
collecting the evidence from practice and analysing the data has promoted choice<br />
and independence for residents and increased the knowledge and skills <strong>of</strong> the care<br />
providers. Over the timeframe there were many challenges that required sustained<br />
effort. <strong>The</strong> introduction <strong>of</strong> the HIQA standards was important and further reinforced<br />
the need to continue this work.<br />
<strong>The</strong> next stage is to sustain the programme <strong>of</strong> work and continue to expand the<br />
knowledge and skill-base <strong>of</strong> the core programme group and extend that growth into<br />
the wider team. Keeping the resident at the centre is critical to the process.<br />
