<strong>The</strong> implementation <strong>of</strong> a model <strong>of</strong> person-centred practice in older person settings<br />

compatible with individual’s values and beliefs and enabling the rights <strong>of</strong> the<br />

residents to a quality <strong>of</strong> life in a caring and respectful environment.<br />

Plans to sustain the work long term<br />

Prior to the completion <strong>of</strong> the programme, the groups identified a number <strong>of</strong> ways to<br />

continue working with some <strong>of</strong> the processes (observation exercises, patient<br />

narratives, high challenge and high support and using Claims Concerns and Issues<br />

for evaluation purposes). <strong>The</strong> group plan to use some <strong>of</strong> the processes in rolling out<br />

the HIQA Standards and keeping the PCC/PD model/framework on all relevant<br />

agendas within their hospitals. <strong>The</strong> internal facilitators have been instrumental in<br />

developing a supportive and challenging climate to implement person-centred care<br />

and promote new ways <strong>of</strong> “thinking”, “being” and “doing” in their sites.<br />
