<strong>The</strong> implementation <strong>of</strong> a model <strong>of</strong> person-centred practice in older person settings<br />

THE NORTH WEST AREA<br />

Two community hospitals participated on the programme from the North West. <strong>The</strong><br />

following section describes each <strong>of</strong> the sites, the practice development groups who<br />

were involved in the programme and a summary <strong>of</strong> their action plans and<br />

achievements.<br />

Participating Sites<br />

St Patrick’s Community Hospital<br />

St Patrick's Community Hospital is situated in Carrick-on-Shannon, County Leitrim.<br />

<strong>The</strong> hospital was built in 1841 and has been used as a care facility since 1928.<br />

<strong>The</strong>re are a range <strong>of</strong> services provided to the local community including, residential<br />

care, respite, a day centre, an X-ray department, physiotherapy and occupational<br />

therapy and specific outpatient clinics. <strong>The</strong> building is located in a residential area on<br />

the periphery <strong>of</strong> the town a short drive <strong>of</strong>f the N4 national Dublin/Sligo roadway.<br />

<strong>The</strong> community hospital has a bed capacity for 112 residents. <strong>The</strong>re are 14 beds<br />

allocated for convalescent care, 10 for respite and one bed for rehabilitation. <strong>The</strong><br />

remaining 87 beds are allocated for long term residential care. Accommodation is<br />

organised over two floors with three units on the ground floor. <strong>The</strong> Sheemore ward is<br />

a 20 bedded female unit, the Dr McGarry, a 28 bedded male unit and the Monsignor<br />

Young an 18 bedded dementia specific unit which was built in 1998. <strong>The</strong><br />

Rivermeade unit on the upper floor provides care for 36 male and female residents.<br />

A nurse’s station and pharmacy area is located on each <strong>of</strong> the units.<br />

<strong>The</strong>re is a reception area located at the entrance foyer with a small c<strong>of</strong>fee shop, and<br />

seating area. A church is also located on the ground floor and is used on a regular<br />

basis by residents, staff and the local community. A quiet room for residents and<br />

their families is also located on the ground floor.<br />

<strong>The</strong> Programme Group – St Patrick’s<br />

<strong>The</strong> group consisted <strong>of</strong> multidisciplinary members as detailed in the table below.<br />

Two internal facilitators – a CNM and a staff nurse - were recruited at the outset, one<br />

worked on the Sheemore ward, and the other worked on the Dr McGarry Ward. One<br />

<strong>of</strong> the internal facilitators (Staff Nurse) left the programme after approximately 8<br />

months and was replaced by an Assistant Director <strong>of</strong> Nursing who was already a<br />
