<strong>The</strong> implementation <strong>of</strong> a model <strong>of</strong> person-centred practice in older person settings<br />

<strong>In</strong>formation from narratives with one resident identified the need to improve end <strong>of</strong><br />

life care practice in the unit. One resident spoke about how he would like to think<br />

that he would be remembered in some way in the unit when he died (Image 2.2). He<br />

felt the unit was very much his home and the staff and residents part <strong>of</strong> his new<br />

family. A multi disciplinary team group with representatives from the key<br />

stakeholders including pastoral care and medical staff was established. An action<br />

plan was developed with specific areas identified for action. Some <strong>of</strong> the<br />

achievements to date include:<br />

• the development <strong>of</strong> a memory book to celebrate the lives <strong>of</strong> residents and<br />

staff who have died was introduced to each <strong>of</strong> the units. Any thoughts, special<br />

moments or events staff, residents and carers would like to recall either in<br />

pictures poems or story to share memories <strong>of</strong> the person are invited for<br />

inclusion in the book.<br />

• introduction <strong>of</strong> the Liverpool Care pathway with staff receiving education on<br />

its implementation from staff who have received training in the UK<br />

• introduction <strong>of</strong> person-centred practices around end <strong>of</strong> life care<br />

• education programmes for staff on palliative care<br />
