<strong>The</strong> implementation <strong>of</strong> a model <strong>of</strong> person-centred practice in older person settings<br />
conducting an observation <strong>of</strong> care. This exercise has been one <strong>of</strong> the most<br />
powerful tools in allowing staff to reflect on practice and how it might feel for the<br />
residents. We have given feedback on the observations <strong>of</strong> care to the team and<br />
any residents who wished to be present. This was a very useful exercise in trying<br />
to get everybody on board and to let them know more about person-centred care.<br />
• Environmental walkabouts have been facilitated and participants encouraged to<br />
involve residents, families and visitors in reflecting on how person-centred the<br />
environment is for people living and working in the setting.<br />
• Participants have been introduced to a reflective framework and encouraged to<br />
undertake reflective exercises about their practice and their facilitation work. It<br />
has been very difficult to get participants to do the reflective work and this is<br />
something that needs to be worked on by the <strong>In</strong>ternal facilitator in future.<br />
• Giving and receiving feedback has improved perceptions <strong>of</strong> the programme as a<br />
supportive and developmental endeavor to improve experiences for both staff<br />
and residents. This has helped to develop the facilitation skills <strong>of</strong> the team.<br />
Specific Action plans Developed across Participating Sites<br />
• Developing meaningful activity plans for the residents.<br />
• Developing a person-centred culture to enhance teamwork and staff relationships<br />
in order to improve communication and well being for staff and residents.<br />
• Enable residents to maintain their connections with family and the outside<br />
community. A meeting was held with the residents group and some family<br />
members in Trim and their issues were very much the same as the themes<br />
emerging from the overall evaluation <strong>of</strong> the programme (Chapter 4). Groups<br />
have been established at the other sites to address family and community<br />
connections.<br />
• Giving residents a voice and choice.<br />
• Working towards making changes and improvements in the environment to<br />
create a person-centred context and culture.<br />
