The implementation of a model of person-centred practice in older person settings

the staff to provide the service
• Hospital Newsletters: capturing the life and stories within the hospitals were
launched with enthusiasm and success. Hospital Information Booklets/Folders
were been developed or reviewed by the participants in all sites with a more
person-centred approach to meeting patient’s and relative’s information needs.
• The participants were actively encouraged to network/liaise with other working
groups/committee members within the hospital i.e. Partnership, Essence of Care,
Ten Steps Programme etc. to share their experiences and learning
• Hospital Day: for both residents/patients and staff were organised
• An unexpected achievement was the final move towards pet therapy -
overcoming the barriers to the implementation of pet therapy has taken place in
one site since the completion of the programme.
