Review of Discussion Points<br />

Say: “Today we learned about two main <strong>to</strong>pics: what questions a person with diabetes should ask his or her<br />

health care provider and how <strong>to</strong> ask them. We also learned the goal levels for A1C, blood pressure,<br />

and cholesterol.<br />

Let’s take something that we learned during the session and make a pledge about food or activity that<br />

we can keep during the month.”<br />

Pledge<br />

Have participants make an individual pledge concerning one objective that they will<br />

work on for the next session. The pledge should be realistic and specifc, and it should<br />

include something that can be maintained over time.<br />

Say: “Our affrmation for this week is ‘To ask well is <strong>to</strong> know much.’ How can this affrmation help you <strong>to</strong><br />

keep your pledge this month.”<br />

Preparation and Reminders for the Next Session<br />

Say: “Next week we’ll be discussing getting your family and friends involved. You may bring<br />

a family member or friend <strong>to</strong> the session if you wish. Just let me know how many people<br />

you’ll be bringing so that we can arrange for enough chairs. Be sure <strong>to</strong> use your GAME<br />

PLAN Food and Activity Tracker this month. And don’t forget <strong>to</strong> use the affrmation <strong>to</strong><br />

help you keep your pledge. We’ll see you next month.”<br />
