Preparing for the Session<br />
NOTE: The NDEP strongly recommends that you invite someone with expertise in physical activity<br />
and diabetes, such as a physical therapist, <strong>to</strong> help conduct this session. Check with local hospitals<br />
and clinics or your state American <strong>Diabetes</strong> Association chapter for suggestions.<br />
Learning Objectives<br />
At the end of this session, participants will be able <strong>to</strong>:<br />
Explain why physical activity is important for helping people with diabetes control their blood<br />
glucose levels, for preventing or delaying diabetes in those at high risk for the disease, and for<br />
helping everyone maintain a healthy weight<br />
Describe easy ways <strong>to</strong> add physical activity <strong>to</strong> your daily routine<br />
OPTIONAL: Identify correct footwear and proper foot care for people with diabetes<br />
Materials, Supplies, Handouts, and Equipment<br />
In addition <strong>to</strong> the general materials, supplies, and equipment listed on page and 2 of the<br />
<strong>Program</strong> Leaders Guide, you will need the following:<br />
Appendix J: GAME PLAN Food and Activity Tracker (one copy per participant)<br />
Appendix K: Weekly Pledge form (one copy per participant)<br />
Original Pre– and Post–Session 3 Questionnaires <strong>to</strong> return <strong>to</strong> participants<br />
Pre– and Post–Session 4 Questionnaires found in Appendix M (one copy per participant, plus a<br />
few extras)<br />
NOTE: Before administering the pre- and post-session questionnaires, check with your sponsoring organization<br />
regarding its guidelines on data collection. Your organization or its funding agencies may require clearance of this<br />
data collection and may have special processes for approval.<br />
