Post–Session 6 Questionnaire<br />
Administer the Post–Session 6 Questionnaire (included in Appendix M). Based on your previous<br />
experience, either read each question and answer choice aloud <strong>to</strong> the group or let the group work in<br />
silence. Suggest that if participants would like assistance because they have forgotten their reading<br />
glasses or for some other reason, you (or your helpers) can assist them. Remind participants <strong>to</strong> write<br />
their names (or their initials with their day and month of birth) on the questionnaires and <strong>to</strong> hand<br />
them in <strong>to</strong> you. Be sure <strong>to</strong> collect all questionnaires so that you can pho<strong>to</strong>copy them and return<br />
them <strong>to</strong> participants at a future session.<br />
Note: Before administering the pre- and post-session questionnaires, check with your sponsoring organization<br />
regarding its guidelines on data collection. Your organization or its funding agencies may require clearance of this<br />
data collection and may have special processes for approval.<br />
Preparation and Reminders for the Next Session<br />
If you had a guest instruc<strong>to</strong>r for this session, consider giving him or her a <strong>to</strong>ken of<br />
your appreciation. It could be an NDEP product (e.g., one of the music CDs: Movimien<strong>to</strong> Por Su<br />
Vida or Step by Step) or something with your organization’s logo.<br />
Be sure <strong>to</strong> send a thank you letter after the session and invite the CDE or HCP <strong>to</strong> Session 2 as a<br />
guest <strong>to</strong> celebrate the completion of the program.<br />
Say: “Next week we’ll be discussing physical activity for families. Be sure <strong>to</strong> use your GAME PLAN Food<br />
and Activity Tracker this week. And don’t forget <strong>to</strong> use the affrmation <strong>to</strong> help you keep your pledge.<br />
We’ll see you next week.”<br />
Note: Tell participants that the <strong>to</strong>pic of session 7 is physical activity and that they will be doing<br />
some extra physical activity during the session. Remind them <strong>to</strong> wear comfortable clothes and<br />
shoes.<br />
