If participants are interested in learning more on their own about diabetes prevention and control,<br />

numbers when fnal formatting in completed). Have them call the <strong>National</strong> <strong>Diabetes</strong> <strong>Education</strong><br />
<strong>Program</strong> (NDEP) at -800-438-5383 <strong>to</strong> order materials. They may also call the public inquiries<br />
line at the Centers for Disease Control and <strong>Prevent</strong>ion (CDC) at -877-CDC-DIAB or the <strong>National</strong><br />
<strong>Diabetes</strong> Information Clearinghouse at -800-860-8747. In addition, interested participants can visit<br />
the NDEP Web site www.ndep.nih.gov <strong>to</strong> download and pho<strong>to</strong>copy materials.<br />
Activity<br />
Take a 5- <strong>to</strong> 0-minute break for physical activity (e.g., walk around the building, down<br />
the hall and back, or in a circle in the room). Try <strong>to</strong> move <strong>to</strong> music, if available.<br />

Note: If this session is conducted in one 90-minute meeting rather than in three separate meetings, be sure <strong>to</strong><br />
review the discussion points from Parts 1 and 2 before beginning this review.<br />
Say: “Today we learned that many circumstances that can affect blood glucose (sugar) levels. We also<br />
learned about Medicare coverage related <strong>to</strong> diabetes equipment and supplies.<br />
Let’s take something that we learned and make a pledge about food or activity that we can keep<br />
during the coming week.”<br />

Have participants make an individual pledge concerning one objective that they will<br />
work on for the next session. The pledge should be realistic and specifc, and it should<br />
include something that can be maintained over time.<br />
Say: “Our affrmation for this week is ‘I am not afraid of s<strong>to</strong>rms because I am learning how <strong>to</strong> sail my<br />
ship.’ How can this affrmation help you <strong>to</strong> keep your pledge this week?”<br />
