Make pho<strong>to</strong>copies of the Post-program Questionnaire found in Appendix M (one for each<br />

participant, plus a few extras).<br />

Order enough copies of Heart-Healthy Home Cooking: African American Style <strong>to</strong> hand out <strong>to</strong><br />

all participants (Appendix AC). Download and print from www.nhlbi.nih.gov/health/index.htm,<br />

or print from the CD-ROM accompanying this curriculum.<br />

Invite any guests (e.g., Tai Chi or yoga instruc<strong>to</strong>rs, registered dietitians, or health care<br />

professionals) who helped with the classes.<br />

Create thank you letters for all of the professionals who gave presentations <strong>to</strong> the group. See<br />

Appendix C: Sample Volunteer Thank You letter for a sample letter.<br />

Bring some basic items for the party (plates, napkins, utensils) in case the participants who<br />

volunteered <strong>to</strong> bring these items forget them.<br />

OPTIONAL: Pho<strong>to</strong>copy the contact sheet that participants flled out at the last session for<br />

distribution at this session.<br />

Conducting the Session<br />

Welcome<br />

Welcome group members and invited guests. Introduce the guests, and allow everyone <strong>to</strong> mingle<br />

for 5 <strong>to</strong> 0 minutes.<br />

Session Overview<br />

Say: “The objectives for <strong>to</strong>day’s session are for us <strong>to</strong> review how small steps in eating and physical activity<br />

have led <strong>to</strong> big rewards for each person in our group.”<br />

Review of the Previous Session<br />

Say: “In the last session, we learned how <strong>to</strong> recognize negative feelings and stress; how <strong>to</strong> cope<br />

with feelings of stress in a positive way, especially by involving others; and how <strong>to</strong> change<br />

negative thoughts in<strong>to</strong> positive thoughts. Before we get started with <strong>to</strong>day’s session, let’s<br />

see how everyone did last week.”<br />

Check-In: Pledge and GAME PLAN Food and Activity<br />

Tracker; Barrier Busters<br />

Facilitate a discussion about how group members did with keeping their pledges and<br />

using their Food and Activity Trackers. Also ask about the barriers they have faced since the last<br />

session. Ask for suggestions from group members on how <strong>to</strong> ‘bust’ these barriers.<br />
