Advance Preparation<br />

In addition <strong>to</strong> the general advance preparation described in the <strong>Program</strong> Leader’s<br />

Resource Guide, you will need <strong>to</strong> do the following:<br />

Make copies of the GAME PLAN Food and Activity Tracker and the Weekly Pledge Form (one<br />

copy per participant).<br />

Make copies of Pre– and Post–Session Questionnaires found in Appendix M (one copy per<br />

participant, plus a few extras).<br />

Make copies of Appendix X: Coping with Bad Feelings and Appendix Y: Stress<br />

Obtain Appendix Z: Talk Back <strong>to</strong> Negative Thoughts (one copy per participant, plus a few extras<br />

<strong>to</strong> share with family members and friends)<br />

Review Appendix G: Troubleshooting for <strong>Program</strong> Leaders <strong>to</strong> prepare for possible interruptions<br />

or challenges in conducting the session.<br />

Make a copy of the sign-up sheet for recording participants’ contributions of food and other items<br />

<strong>to</strong> the celebration party at the fnal session (found at the end of this lesson plan).<br />

OPTIONAL: Order or download and pho<strong>to</strong>copy the NDEP publications Dealing with the Ups<br />

and Downs of <strong>Diabetes</strong> and Tips for Helping a Person with <strong>Diabetes</strong>. See information in preceding<br />

section.<br />

This session may reveal that some participants would like <strong>to</strong> seek counseling. Unless you are a<br />

trained mental health professional, you should not attempt <strong>to</strong> provide counseling. Research the<br />

availability of mental health services in your community so that if the need arises, you can make<br />

an immediate recommendation for a participant seeking mental health counseling or advice about<br />

emotional needs<br />

Welcome<br />

Say: “Welcome back, everyone. Today we’re going <strong>to</strong> talk about coping with stress and changing negative<br />

thoughts in<strong>to</strong> positive ones. But frst, I’d like <strong>to</strong> know if any of you have eaten at a restaurant since<br />

the last session and, if so, whether you used the tips we talked about two sessions ago. Does anyone<br />

have a s<strong>to</strong>ry <strong>to</strong> share?”<br />

Allow 5 <strong>to</strong> 0 minutes for sharing s<strong>to</strong>ries and tips. If no one volunteers, be prepared <strong>to</strong> share a s<strong>to</strong>ry<br />

of your own.<br />
