Advance Preparation<br />

In addition <strong>to</strong> the general advance preparation described in the <strong>Program</strong> Leader’s<br />

Resource Guide, you will need <strong>to</strong> do the following:<br />

Pho<strong>to</strong>copy the GAME PLAN Food and Activity Tracker (Appendix J) and the Weekly Pledge<br />

form (Appendix K) (one copy per participant). (NOTE: not needed if Session 6 is given as one<br />

90-minute session.)<br />

Make enough copies of the three brochures in Appendix R: Blood Sugar Testing: The <strong>Power</strong> <strong>to</strong><br />

Control <strong>Diabetes</strong> Is in Your Hands for each member of the group, plus some extras. Order these<br />

brochures using the NDEP Publications Order Form in Appendix AC, download and print them<br />

from the NDEP Web site at http://www.ndep.nih.gov/campaigns/<strong>Power</strong>/<strong>Power</strong>_index.htm, or<br />

print them from the CD-ROM accompanying this curriculum. If ordering, allow at least 6 weeks<br />

for delivery.<br />

Make copies of Appendix Y: Stress for each participant, plus some extras.<br />

Pho<strong>to</strong>copy the Post–Session 6 Questionnaire (one per participant, plus some extras).<br />

Review Appendix G: Troubleshooting for <strong>Program</strong> Leaders <strong>to</strong> prepare for possible interruptions<br />

or challenges in conducting the session.<br />

Explain why controlling diabetes is important<br />

List circumstances that can affect blood glucose (sugar) levels<br />

Describe the basics of Medicare coverage related <strong>to</strong> diabetes equipment and supplies<br />

NOTE: If you are conducting Session 6 in a single 90-minute meeting, skip ahead NOW <strong>to</strong> Part 3 Discussion<br />

Points (List page when fnal formatting is completed) If the session is being held in three 60-minute meetings,<br />

proceed through this lesson plan as written.<br />

Review of the Previous Session<br />

Say: “In the last session, we learned about the ABCs of diabetes: A1C, blood pressure, and cholesterol.<br />

Before we get started with <strong>to</strong>day’s session, let’s see how everyone did last week.”<br />
