Give each participant several copies of the Weekly Pledge form and the GAME PLAN Food and<br />

Activity Tracker <strong>to</strong> use after the program concludes.<br />

Review of Pre– and Post–Session 11 Questionnaires<br />

Return the Pre– and Post–Session Questionnaires <strong>to</strong> participants. Ask the group if anyone<br />

has questions or would like <strong>to</strong> discuss these questionnaires. Explain that the purpose of the<br />

questionnaires is two-fold: ) <strong>to</strong> give you as program facilita<strong>to</strong>r feedback on how confdent people<br />

feel with the new information and skills you are trying <strong>to</strong> help them build and 2) <strong>to</strong> help them<br />

refect on what progress they are making and where they may want <strong>to</strong> ask for a helping hand.<br />

Post-program Questionnaire<br />

Hand out the Post-program Questionnaire. Based on your previous experience, either read each<br />

question and answer choice aloud <strong>to</strong> the group or let the group work in silence. Suggest that if<br />

participants would like assistance because they have forgotten their reading glasses or for some<br />

other reason, you (or your helpers) can assist them. Remind participants <strong>to</strong> write their names (or<br />

their initials with their day and month of birth) on the questionnaires and <strong>to</strong> hand them in <strong>to</strong> you.<br />

Be sure <strong>to</strong> collect all questionnaires so that you can pho<strong>to</strong>copy them and return them <strong>to</strong> participants<br />

by mail. If you do not have all of the participants’ mailing addresses, request that contact<br />

information now by explaining that you need it <strong>to</strong> return the questionnaires. You can distribute<br />

copies of Appendix I: Sign-In and Contact Form.<br />

Discussion Points<br />

Ask group members <strong>to</strong> give a brief testimony of how the program helped improve<br />

their health. Ask them <strong>to</strong> state whether they achieved their goals and which sessions<br />

they found most useful. Have group members state whether they were able <strong>to</strong> get their family<br />

members and friends involved in the lifestyle changes. Also invite family members or friends <strong>to</strong><br />

comment.<br />

OPTIONAL: Take brief notes during the testimonials so that later you can say a few words<br />

acknowledging the group members who shared testimonials.<br />

Give out the thank you letters <strong>to</strong> the diabetes professionals and other professionals who came as<br />

guest instruc<strong>to</strong>rs <strong>to</strong> one or more group sessions (or mail them later).<br />

Activity<br />

Take a 5- <strong>to</strong> 0-minute break for physical activity (e.g., walk around the building, down<br />

the hall and back, or in a circle in the room). Try <strong>to</strong> move <strong>to</strong> music, if available.<br />
