Individual Sessions<br />

The program consists of 2 sessions. Session 6 (<strong>Diabetes</strong> Overview, Parts –3) can be conducted<br />

in a single session or divided in<strong>to</strong> 3 sessions (bringing the <strong>to</strong>tal <strong>to</strong> 4 sessions), depending on the<br />

Describe the impact that small steps can have in preventing or delaying progression <strong>to</strong> type 2<br />

diabetes and in controlling diabetes<br />

Use <strong>to</strong>ols <strong>to</strong> help take small steps in food choices and in physical activity levels<br />

Identify small steps that can lead <strong>to</strong> big rewards<br />

Explain that there are more than 50 ways <strong>to</strong> prevent diabetes<br />

Use <strong>to</strong>ols <strong>to</strong> help take small steps in food choices and in physical activity levels<br />

Describe the importance of choosing more healthful foods<br />

Identify more healthful foods by reading food labels<br />

Choose <strong>to</strong> incorporate healthier eating in<strong>to</strong> daily routines<br />

Explain why physical activity is important for helping people with diabetes control their blood<br />

glucose levels, for preventing or delaying diabetes in those at high risk for the disease, and<br />
