Post–Session 4 Questionnaire<br />
Administer the Post–Session 4 Questionnaire. Remind participants <strong>to</strong> write their names (or their<br />
initials with their day and month of birth) on the questionnaires and <strong>to</strong> hand them in <strong>to</strong> you.<br />
Preparation and Reminders for the Next Session<br />
No optional activities are designed for Session 5, but you might choose <strong>to</strong> add an optional activity<br />
from Session 3 or Session 4. Reviewing the responses on the pre–Session Questionnaires can help<br />
you <strong>to</strong> select an activity that meets the interests and needs of the group.<br />
If you will be adding an optional activity at the next session, remind participants that if they wish<br />
<strong>to</strong> participate in that activity, they will need <strong>to</strong> stay longer (at least 2 hours).<br />
If the optional activity involves traveling somewhere, ask for volunteers for carpooling, or discuss<br />
other arrangements for transporting participants <strong>to</strong> the site of the activity.<br />
Ask participants if they are willing <strong>to</strong> prepare and bring some healthful foods and the recipes for<br />
these foods <strong>to</strong> share at the next session.<br />
This is the time <strong>to</strong> ask participants how interested they are in learning more about diabetes.<br />
From your review of the pre-program questionnaires from the frst session, and from your<br />
interactions with the group during the past four sessions, you probably have a “feel” for how<br />
much information the group would like on diabetes. Are most people participating in the <strong>Power</strong> <strong>to</strong><br />
<strong>Prevent</strong> program <strong>to</strong> make general healthy improvements in their lifestyle, or are they specifcally<br />
interested in diabetes? How many people in the group have diabetes?<br />
Ask whether the group as a whole wants a one-session general overview of diabetes or a more<br />
detailed, three-session class. Try <strong>to</strong> reach a consensus by getting the whole group <strong>to</strong> agree instead<br />
of putting the question <strong>to</strong> a vote. This decision will determine whether you conduct Session 6 in<br />
one meeting or over three (or more) meetings.<br />
NOTE: Session 6 must be taught by a trained health care professional (e.g., a certifed diabetes<br />
educa<strong>to</strong>r or physician). Because this guest instruc<strong>to</strong>r will need advance notice, you must plan ahead<br />
and ask now about the interests of the group. The NDEP recommends contacting local clinics or your<br />
local chapter of the American <strong>Diabetes</strong> Association or the American Association of <strong>Diabetes</strong> Educa<strong>to</strong>rs<br />
for possible volunteers. See Session 6 for further suggestions.<br />
Say: “Next week we’ll be discussing how <strong>to</strong> add healthy eating <strong>to</strong> your daily routine. Please<br />
bring one food item that you prepare regularly <strong>to</strong> next week’s class, and also bring the<br />
recipe. Be sure <strong>to</strong> use your GAME PLAN Food and Activity Tracker this week. I’d be<br />
glad <strong>to</strong> stay and talk one-on-one with anyone having trouble using the tracker.<br />
Remember, “Small Steps. Big Rewards”! And don’t forget <strong>to</strong> use the affrmation <strong>to</strong> help<br />
you keep your pledge. We’ll see you next week.”<br />
